Half of US school children receive influenza vaccine. In our previous trials, school-located influenza vaccination (SLIV) raised vaccination rates by 5 to 8 percentage points. We assessed whether text message reminders to parents could raise vaccination rates above those observed with SLIV.

Does Patient-centered Care Change Genital Surgery Decisions? The Strategic Use of Clinical Uncertainty in Disorders of Sex Development Clinics.

Genital surgery in children with ambiguous or atypical genitalia has been marred by controversies about the appropriateness and timing of surgery, generating clinical uncertainty about decision making. Since 2006, medical experts and patient advocates have argued for putting the child's needs central as patient-centered care. Based on audio recordings of 31 parent-clinician interactions in three clinics of disorders of sex development, we analyze how parents and clinicians decide on genital surgery.

Measuring Value in Internal Medicine Residency Training Hospitals Using Publicly Reported Measures.

Graduate medical education (GME) lacks measures of resident preparation for high-quality, cost-conscious practice. The authors used publicly reported teaching hospital value measures to compare internal medicine residency programs on high-value care training and to validate these measures against program director perceptions of value. Program-level value training scores were constructed using Centers for Medicare & Medicaid Services Value-Based Purchasing (VBP) Program hospital quality and cost-efficiency data.

Systems Opportunities to Reduce ED Crowding From Nonemergency Referrals.

Emergency department (ED) crowding threatens patient safety and is associated with increased mortality. This study explored the role of nonurgent referrals to the ED in crowding and collaborated on a large quality initiative with the study institution's accountable care organization (ACO) to provide timely alternatives to such referrals. Fifty-two percent of nonemergent ED patients report contacting a medical provider prior to coming to the ED, with 70% of those providers directing the patient to go to the ED.

Evaluating the Hierarchical Structure of ADHD Symptoms and Invariance Across Age and Gender.

The bifactor model of attention-deficit/hyperactivity disorder (ADHD) has been extensively explored, yet the tendency of the bifactor model to overfit data necessitates investigation of alternative, more parsimonious models, such as a modified bifactor structure. The present study used item response theory to compare unidimensional, correlated factors, bifactor, and modified bifactor models of ADHD symptoms in a clinical sample of youth ( N = 1,612) and examined differential item functioning (DIF) by age (<11 and ≥11 years) and gender. Results suggested that two restricted bifactor models showed superior fit compared with alternative models, and support strong general and inattention dimensions, with unreliable hyperactivity and impulsivity dimensions.

Nor have improvements in skills been as clinically robust as one might hope. We review some possible explanations, then potential technology-enabled solutions.

Dangers, Diversions, and Decisions: The Process of Criminal Desistance Among Formerly Incarcerated Young Men.

Young adult men (ages 18-24) who exit the juvenile justice system are at high risk for repeat offending. However, little is known qualitatively about the strategies that they use to navigate criminal influences, crime temptations, and the possibility of getting "caught." To address this gap, the authors used narrative methods involving 30 in-depth qualitative interviews with 15 formerly incarcerated young men between the ages of 19 and 24.

PD-1 blockade induces responses by inhibiting adaptive immune resistance.

Therapies that target the programmed death-1 (PD-1) receptor have shown unprecedented rates of durable clinical responses in patients with various cancer types. One mechanism by which cancer tissues limit the host immune response is via upregulation of PD-1 ligand (PD-L1) and its ligation to PD-1 on antigen-specific CD8(+) T cells (termed adaptive immune resistance). Here we show that pre-existing CD8(+) T cells distinctly located at the invasive tumour margin are associated with expression of the PD-1/PD-L1 immune inhibitory axis and may predict response to therapy.

A Sociobehavioral Model of Use of Complementary and Alternative Medicine Providers, Products, and Practices: Findings From the 2007 National Health Interview Survey.

This study tested a modification of the Andersen behavioral model of health services use to assess the effects of predisposing factors, enabling resources, need, and personal health practices on the use of complementary and alternative medicine providers, products, and practices in the past 12 months. Data were from the 2007 National Health Interview Survey (n = 23 149 adults). Prevalence estimates and logistic regression results were weighted and adjusted for complex sample design.
