Coronary microvascular dysfunction (CMD) comprises a wide spectrum of structural and/or functional abnormalities of coronary microcirculation that can lead to myocardial ischemia. Emerging evidence has indicated that CMD is a relevant cause of morbidity and mortality and is associated with a high risk of major adverse cardiovascular events (MACEs) and heart failure with preserved ejection fraction as well as poor quality of life. This case report presents a complex and challenging scenario of recurrent () bacteremia and tricuspid valve endocarditis in a 77-year-old male patient with multiple comorbidities and indwelling medical devices. The patient's medical history was significant for T4 paraplegia, neurogenic bladder requiring a chronic indwelling suprapubic catheter, heart block status post-permanent pacemaker placement, type 2 diabetes mellitus, chronic kidney disease, and chronic sacral wounds. The case highlights the difficulties in managing antibiotic-resistant infections, particularly in patients with implantable devices and chronic wounds.

Alcohol use disorders can cause peripheral and central neurological disorders with symptoms such as pain, numbness, paresthesia, and dysesthesia, often impairing walking ability. However, effective treatments for alcohol-related peripheral neuropathy are yet to be identified. This case report highlights the successful use of dysesthesia-matched transcutaneous electrical nerve stimulation (DM-TENS) and aerobic exercise in a 53-year-old woman with alcohol-related peripheral neuropathy who presented with severe pain and walking difficulties.
