Management of hypocalcaemia in the critically ill.

Curr Opin Crit Care

August 2023

Department of Intensive Care Medicine, Gelderse Vallei Hospital, Ede.

Purpose Of Review: To better understand the established associations between hypocalcaemia and clinical outcomes, we synopsize the mechanisms involved in hypocalcaemia in the critically ill. We also provide an overview of the current evidence on managing hypocalcaemia in critical illness.

Recent Findings: Hypocalcaemia is reported to occur in 55-85% of ICU patients.

Background: Critically ill patients are subject to severe skeletal muscle wasting during intensive care unit (ICU) stay, resulting in impaired short- and long-term functional outcomes and health-related quality of life. Increased protein provision may improve functional outcomes in ICU patients by attenuating skeletal muscle breakdown. Supporting evidence is limited however and results in great variety in recommended protein targets.

Article Synopsis
  • Abdominal complaints (AC) are common in runners, with 57% reporting issues during or after running, often impacting performance.
  • Factors like female gender, younger age, and high-intensity running are linked to increased AC prevalence.
  • Dietary habits, especially higher intake of fats, tea, and unhealthy foods, have associations with AC, particularly in men with lower abdominal complaints.

Background: The role of radiological staging and surveillance imaging is under debate for T1 colorectal cancer (CRC) as the risk of distant metastases is low and imaging may lead to the detection of incidental findings.

Objective: The aim of this study was to evaluate the yield of radiological staging and surveillance imaging for T1 CRC.

Methods: In this retrospective multicenter cohort study, all patients of 10 Dutch hospitals with histologically proven T1 CRC who underwent radiological staging in the period 2000-2014 were included.

Is Behavioural Therapy a New Treatment Option for Task-Specific Dystonia in Athletes? A Case Series.

Tremor Other Hyperkinet Mov (N Y)

May 2023

Center for Human Movement Sciences, University Medical Center Groningen, University of Groningen and Department Sports and Exercise Medicine, Sports Valley, Gelderse Vallei Hospital, Ede, The Netherlands.

Background: Task-specific dystonia is a movement disorder of the central nervous system characterized by focal involuntary spasms and muscle contractions, which can negatively affect performance of a specific task. It can affect a wide range of fine motor skills, also in athletes. Current management of task-specific dystonia includes mainly prescribing drugs, exercise therapy or botulinum injections to the affected muscles.

Objectives: Lower limb tendinopathy (LLT) is highly prevalent in runners. Treatment can be challenging, and knowledge of risk factors may be valuable to develop preventive or treatment interventions for LLT. The aims of this study were (1) to assess the prevalence of three common LLTs (Achilles tendinopathy (AT), patellar tendinopathy and plantar fasciopathy) in a large cohort of Dutch and Belgian runners and (2) to investigate its association with potential risk factors, with a particular focus on nutritional factors in the habitual diet.

Objectives: Translating the Oral Health Assessment Tool (OHAT) into Dutch and assessing the inter-rater reliability among community nurses.

Methods: The original version of the OHAT was translated following the five stages of the 'Guidelines for the Process of Cross-Cultural Adaptation'. After a forward and backward translation, consensus on the pre-final version was obtained in an expert panel discussion.

Article Synopsis
  • The DATA study assessed the impact of extending anastrozole treatment to either 3 or 6 years for women with hormone receptor-positive breast cancer after they had been disease-free for 2-3 years post-tamoxifen.
  • The research, conducted in 79 Dutch hospitals, followed 1912 patients who were randomly assigned to one of the two treatment durations, examining their disease-free and overall survival rates over a minimum of 10 years.
  • Results showed that while the 6-year treatment group had a slightly better disease-free survival rate (69.2% vs. 66.0%), the difference was not statistically significant, suggesting extending treatment did not significantly improve outcomes for these patients.

Background: Advance Care Planning (ACP) enables patients to define and discuss their goals and preferences for future medical treatment and care. However, the structural implementation of ACP interventions remains challenging. The Multidisciplinary Timely Undertaken Advance Care Planning (MUTUAL) intervention has recently been developed which takes into account existing barriers and facilitators.

Objective: Routine urgent endoscopic retrograde cholangiopancreatography (ERCP) with endoscopic biliary sphincterotomy (ES) does not improve outcome in patients with predicted severe acute biliary pancreatitis. Improved patient selection for ERCP by means of endoscopic ultrasonography (EUS) for stone/sludge detection may challenge these findings.

Design: A multicentre, prospective cohort study included patients with predicted severe acute biliary pancreatitis without cholangitis.

Article Synopsis
  • A dedicated lifestyle front office (LFO) can enhance patient-centered lifestyle care by connecting healthcare with community-based initiatives, crucial for preventing noncommunicable diseases.
  • The LOFIT study will conduct two randomized controlled trials in the Netherlands focused on patients with (cardio)vascular and musculoskeletal disorders, comparing an intervention group receiving lifestyle coaching to a usual care control group.
  • The primary outcome will measure health risk and lifestyle factors through a composite score, while secondary outcomes will assess various health markers and patient behaviors.

Purpose: COVID-19 associated pulmonary aspergillosis (CAPA) is associated with increased morbidity and mortality in ICU patients. We investigated the incidence of, risk factors for and potential benefit of a pre-emptive screening strategy for CAPA in ICUs in the Netherlands/Belgium during immunosuppressive COVID-19 treatment.

Materials And Methods: A retrospective, observational, multicentre study was performed from September 2020-April 2021 including patients admitted to the ICU who had undergone diagnostics for CAPA.

Purpose Of Review: To summarize recent research on critical care nutrition focusing on the optimal composition, timing, and monitoring of enteral feeding strategies for (post)-ICU patients. We provide new insights on energy and protein recommendations, feeding intolerance, and describe nutritional practices for coronavirus disease 2019 ICU patients.

Recent Findings: The use of indirect calorimetry to establish individual energy requirements for ICU patients is considered the gold standard.

Introduction: Patients with early-stage and locally advanced rectal cancer are often treated with neoadjuvant therapy followed by surgery or watch and wait. This study evaluated the role of circulating tumor DNA (ctDNA) to measure disease after neoadjuvant treatment and surgery to optimize treatment choices.

Materials And Methods: Patients with rectal cancer treated with both chemotherapy and radiotherapy were included and diagnostic biopsies were analyzed for tumor-specific mutations.

Background: Patients discharged from the Intensive Care Unit (ICU) frequently suffer from ICU-acquired weakness because of immobilization and massive inflammation-induced muscle mass loss. Consequently, rehospitalization, reduced quality of life (QoL), increased disabilities, and higher post-ICU mortality is observed. Exercise rehabilitation and optimal nutrition, particularly protein intake, are pivotal to regaining muscle mass and function.

Introduction And Hypothesis: Among women worldwide, pelvic organ prolapse (POP) is a common problem. There are three different treatment options for POP: pelvic floor muscle therapy, pessary treatment and prolapse surgery. As none of the three treatment options is clearly superior, shared decision making (SDM) is very important.

Background: Although the Netherlands is a country with a low endemic level of methicillin-resistant (MRSA), a national MRSA surveillance has been in place since 1989. In 2003 livestock emerged as a major reservoir of MRSA and currently livestock-associated MRSA (clonal complex CC398) make up 25% of all surveillance isolates. To assess possible transfer of resistant strains or resistance genes, MRSA obtained from humans and animals were characterized in detail.
