Immunotherapy "Shock" with vitiligo due to nivolumab administration as third line therapy in lung adenocarcinoma.

Respir Med Case Rep

October 2017

Sana Clinic Group Franken, Department of Cardiology/Pulmonology/Intensive Care/Nephrology, "Hof'' Clinics, University of Erlangen, Hof, Germany.

Non-small cell lung cancer is still diagnosed at late stage due to the lack of early symptoms and methods of diagnostic prevention. In the past ten years several targeted therapies have been introduced or explored. Tyrosine kinase inhibitors and immunotherapy are currently considered the most effective and safe therapies in comparison to the non-specific cytotoxic agents.

Background: Interstitial lung diseases (ILD) are chronic and restrictive lung diseases with poor survival and quality of life. The aim of this study was to investigate the frequency of sleep disorders in idiopathic pulmonary fibrosis (IPF) and sarcoidosis and to assess patients' quality of life in relation to these disorders.

Methods: Forty patients, 19 with IPF, and 21 with sarcoidosis stage II/III were included.

Sana Clinic Group Franken, Department of Cardiology/Pulmonology/Intensive Care/Nephrology, "Hof" Clinics, University of Erlangen, Hof, Germany.

In this decade a "bloom" of novel therapies has been observed for non-small cell lung cancer. We have new tools for the diagnosis of lung cancer and also we can re-biospy easier than before in different lesions and obtain tissue samples in order to investigate whether a patient can receive new targeted therapies. Immunotherapy has been well established previously for other forms of cancer, and nowadays it is also available for lung cancer.

Endobronchial ultrasound convex probe for lymphoma, sarcoidosis, lung cancer and other thoracic entities. A case series.

Respir Med Case Rep

August 2017

Sana Clinic Group Franken, Department of Cardiology/Pulmonology/Intensive Care/Nephrology, "Hof'' Clinics, University of Erlangen, Hof, Germany.

Endobronchial ultrasound endoscopy is a state of the art diagnostic endoscopic procedure for the thorax. Firstly it was designed mainly for the staging of lung cancer and of course for the diagnosis of suspicious findings in large central airways. The main limitation of the equipment is the diameter of the instrument and therefore it can only be guided through large airways.

Possible adverse effects of immunotherapy in non-small cell lung cancer; treatment and follow-up of three cases.

Respir Med Case Rep

July 2017

Sana Clinic Group Franken, Department of Cardiology / Pulmonology / Intensive Care / Nephrology, "Hof" Clinics, University of Erlangen, Hof, Germany.

Article Synopsis
  • Over the last ten years, new treatments for non-small cell lung cancer adenocarcinoma have emerged based on pharmacogenomics, highlighting the importance of specific genetic tests in everyday clinical practice.
  • Key genetic factors to investigate include mutations in the epidermal growth factor receptor, anaplastic lymphoma kinase, and programmed death-ligand 1 to guide therapy choices for cancer patients.
  • The case series will discuss the adverse effects experienced by patients treated with the immunotherapies nivolumab and pembrolizumab, along with insights on their treatment and follow-up care.

"Liquid elbows" due to afatinib administration.

Respir Med Case Rep

June 2017

Sana Clinic Group Franken, Department of Cardiology/Pulmonology/Intensive Care/Nephrology, "Hof" Clinics, University of Erlangen, Hof, Germany.

Non-small cell lung cancer adenocarcinoma in the past decade has targeted therapies as the cornerstone for therapy. In specific patients with epidermal growth factor receptor mutation have three different therapy approaches with the tyrosine kinase inhibitors: erlotinib, gefitinib and afatinib. Nowadays we can use tyrosine kinase inhibitors as second line treatment for squamous cell carcinoma.

Thoracic duct cysts of the mediastinum are extremely rare entities and their pathogenesis still remains unknown. Imaging methods are not specific and show a cystic mass, however the real nature of the lesion is confirmed only with the help of histopathological examination after surgical excision. Here, we present a case of thoracic cyst in a 28-year-old female, lining in posterior lower mediastinum.

Article Synopsis
  • Non-small cell lung cancer (NSCLC) is the most common lung cancer, with 60% of patients diagnosed having advanced disease; this study focused on long-term survival factors in those with unresectable NSCLC.
  • A total of 1,156 patients diagnosed with advanced NSCLC (stage IIIB or IV) were analyzed, revealing a mean age of 62 and that males were more prevalent; adenocarcinoma was the most common type, especially among women.
  • Significant survival factors identified included weight loss, histological type, treatment combinations, and the benefit of radio lung treatment, while certain distant metastases and the use of a third drug showed no positive impact on survival outcomes.

Aurora kinases (AKs) represent a novel group of serine/threonine kinases. They were originally described in 1995 by David Glover in the course of studies of mutant alleles characterized with unusual spindle pole configuration in Drosophila melanogaster. Thus far, three AKs A, B, and C have been discovered in human healthy and neoplastic cells.

Article Synopsis
  • Recent studies have shown that specific genetic mutations in NSCLC, such as EGFR and ALK, can be targeted with tyrosine kinase inhibitors (TKIs), leading to significantly improved response rates and progression-free survival compared to traditional chemotherapy.
  • Current trials are exploring the potential benefits of targeted therapies in early-stage NSCLC, aiming to improve outcomes and minimize side effects, with results still pending from ongoing studies.

Atrial fibrillation (AF) is consider to be the most common cardiac arrhythmia with an increasingly prevalence. It is postulated that the source of thromboembolism in 90% of patients with non-valvular AF arises from the left atrial appendage (LAA). Stroke is the most feared and life threatening consequence of thromboembolism.

Right heart failure (RHF) is a frequent complication following left ventricular assist device (LVAD) implantation. The incidence of RHF complicates 20-50% (range, 9-44%) of cases and is a major factor of postoperative morbidity and mortality. Unfortunately, despite the fact that many risk factors contributing to the development of RHF after LVAD implantation have been identified, it seems to be extremely difficult to avoid them.

MV regurgitation occurs with the increasing frequency of degenerative changes of the aging process. Moreover, other causes of clinically significant MV regurgitation include cardiac ischemia, infective endocarditis and rhematic disease more frequently in less developed countries.

The criteria will be the symptoms, the evidence of ischemia and the anatomical complexity of the coronary bed. Main indications are persistence of symptoms, despite oral medical treatment and the prognosis of any intervention.
