Endocarditis and Hypertrophic Cardiomyopathy.

JACC Case Rep

March 2025

Division of Cardiology, Department of Translational Medical Sciences, University of Campania Luigi Vanvitelli, Naples, Italy; Department of Cardiology, Umberto I Hospital, Nocera Inferiore, Italy.

Infective endocarditis (IE) is an uncommon but potentially fatal complication in patients affected by hypertrophic cardiomyopathy (HCM). The risk has been described to be significantly higher than in the general population, but the incidence of IE in HCM population remains unknown. The complex pathophysiology of this disease, characterized by structural alterations of the mitral valve apparatus and the presence of turbulent flow that promotes the deposition of microorganisms, could provide a substrate for IE and may, to some extent, explain its higher incidence in this specific population.

Genotyping and treatment issues with 'unusual' HCV 1 subtypes.

Eur J Clin Microbiol Infect Dis

February 2025

Department of Medical Biotechnologies, University of Siena, Siena, Italy.

Unusual non 1a/1b HCV 1 subtypes are mostly prevalent in Sub-Saharan Africa. These variants harbour resistance associated substitutions as natural polymorphisms in the NS5A region and are difficult to characterize and treat. We report the HCV genome sequences from two African subjects with unusual subtype 1 g and 1e treated with the pangenotypic regimen sofosbuvir/velpatasvir with failure in the latter.

Background: Cardiac arrest (CA) is a leading cause of mortality and morbidity, with survivors often developing post-cardiac arrest syndrome (PCAS), characterized by systemic inflammation, ischemia-reperfusion injury (IRI), and multiorgan dysfunction. Acute kidney injury (AKI), a frequent complication, is associated with increased mortality and prolonged intensive care unit (ICU) stays. This study evaluates AKI incidence and progression in cardiac arrest patients managed with different temperature protocols and explores urinary biomarkers' predictive value for AKI risk.

Background: Complete revascularization has been shown to be superior to culprit-only treatment in patients with ST-segment elevation myocardial infarction (STEMI) and multivessel disease. However, it remains unclear whether complete revascularization should be guided by coronary physiology or conventional angiography. Angiography-derived physiology may allow functional assessment and procedural guidance using angiograms from primary percutaneous coronary intervention (PCI), potentially maximizing the benefits of a physiology-guided approach.
