Article Synopsis
  • Safety studies have shown that new anticoagulant drugs are at least as safe as older ones like warfarin, but emergency data on bleeding symptoms suggest discrepancies with existing research.
  • The study aimed to explore how often patients on different anticoagulants visit the emergency department due to bleeding and related symptoms.
  • The findings indicate that a significant portion of warfarin patients were inadequately monitored, with bleeding rates that were slightly higher compared to those on direct oral anticoagulants, highlighting the need for better tracking of bleeding issues in real-life scenarios.

Endoplasmic reticulum (ER) stress, prompted by the accumulation of misfolded or unfolded proteins, triggers the activation of the unfolded protein response (UPR) pathway to restore ER homeostasis. This stress response is implicated in the development of hepatocellular carcinoma (HCC). A biallelic mutation in SPRTN is currently the only known single-gene mutation implicated in the early onset of HCC.

  • Buerger's disease (BD) is a serious condition that requires early diagnosis for effective treatment, but there’s no universally accepted method for diagnosing it due to varied criteria used by different vascular centers.
  • A recent Delphi Consensus Study highlighted the lack of consensus on BD diagnostic criteria, particularly beyond the requirement of a history of smoking, making it hard to compare patient outcomes globally.
  • The VAS-European Independent Foundation has proposed that a definitive BD diagnosis should include a history of smoking, typical angiographic and histopathological features, and suggests using a combination of major and minor criteria for suspected diagnoses, with validation studies currently in progress.

The increasing prevalence of diabetes mellitus, obesity, and metabolic syndrome in the population can lead to metabolic dysfunction-associated steatohepatitis (MASH) and metabolic dysfunction-associated steatotic liver disease (MASLD). In Western industrialized countries, this has become a major problem with significant socioeconomic impacts. MASH is now a leading cause of liver transplantation (LT), especially in developed countries.

Fragility spinal fractures among cirrhotic liver transplant candidates in Croatia.

Injury

February 2024

Division of general and sport traumatology and orthopaedics, Department of surgery, Merkur University Hospital, Zajčeva 19, 10000 Zagreb, Croatia; School of Medicine, University of Zagreb, Šalata 3, 10000 Zagreb, Croatia.

Introduction: Existing data on fragility spinal fractures prevalence in liver transplant candidates are scarce and inconsistent. This may be due to other comorbidities, besides hepatic osteodystrophy (HO), that contribute to bone loss and fragility fracture prevalence in chronic liver disease (CLD).

Objectives: The aim of this study was to investigate the prevalence of spinal thoracic and lumbar fragility fractures among cirrhotic, non-chronic kidney disease (CKD), non-diabetic liver transplant candidates and to explore their relationship with clinical characteristics, laboratory markers and dual-energy x-ray absorptiometry (DXA) results.

Fibroblast growth factor 23 and its role in bone diseases.

Growth Factors

February 2024

Department of Surgery, Division of General and Sport Traumatology and Orthopaedics, Merkur University Hospital, Zagreb, Croatia.

Fibroblast growth factor 23 (FGF23) has been casually linked to numerous hypophosphatemic bone diseases, however connection with bone loss or fragility fractures is still a matter of debate. The purpose of this review is to explore and summarise the known actions of FGF23 in various pathological bone conditions. Besides implication in bone mineralisation, elevated FGF23 showed a pathological effecton bone remodelling, primarily by inhibiting osteoblast function.

Fluoroquinolones are a commonly prescribed class of antibiotics due to their broad spectrum of antimicrobial activity, favorable pharmacokinetic properties, ability to switch from parenteral to oral administration, and global availability. After beta-lactams, they are the second most common antibiotic class associated with drug allergies. The mechanism of fluoroquinolone-induced hypersensitivity reactions has not yet been fully understood, so the true incidence of hypersensitivity reactions remains unknown.

Article Synopsis
  • Croatia has seen a rise in various vector-borne diseases, including several flaviviruses like TBEV, WNV, and USUV in the continental areas, while Toscana virus and sandfly fever viruses are found along the coast.
  • Sporadic cases of infections such as TAHV and Bhanja bandavirus have emerged, alongside established diseases like Lyme borreliosis, which is prevalent due to high tick populations, and rickettsial infections like Mediterranean spotted fever.
  • Despite the eradication of malaria in 1964, only imported cases have been observed, highlighting the need for ongoing monitoring of vector populations to mitigate risks of increased vector-borne disease transmission. *

Aims: Our study aimed to investigate the relationship between three autoantibodies and their combination with anthropometric and metabolic components and microvascular complications in patients with latent autoimmune diabetes in adults (LADA).

Methods: Our study included 189 LADA patients divided into four subgroups according to the autoantibodies present: glutamic acid decarboxylase autoantibodies (GADA) only; zinc transporter-8 autoantibodies (ZnT8A)+GADA; insulinoma-associated-2 autoantibodies (IA-2)+GADA; and ZnT8+IA-2+GADA.

Results: Compared to GADA positivity only, patients with ZnT8+GADA positivity and ZnT8+IA-2+GADA positivity had a shorter diabetes duration and lower body mass index (BMI); patients with ZnT8+GADA positivity were younger and showed an increase in glomerular filtration rate, while those with ZnT8+IA-2+GADA positivity had lower C-peptide and lower insulin resistance measured with HOMA2-IR.

Article Synopsis
  • - A study analyzing 165 multiple myeloma (MM) patients found that those who developed second primary malignancies (SPMs), particularly hematological ones, generally had a shorter survival time and were more likely to have had previous cancer treatments.
  • - Patients with hematological SPMs tended to be younger and had a higher rate of prior autologous hematopoietic cell transplantation, with treatment often stopped upon SPM detection, especially in cases of hemato-SPM.
  • - Overall survival (OS) post-SPM detection was only 8.5 months, with the main cause of death being the SPM; hence, regular monitoring and early intervention are crucial for improving patient outcomes.
