The Gamblegram consists of two bars, each of which represents the sum of the charges of individual positively and negatively charged ions and is commonly used for visualizing changes in acid-base and electrolyte charges. However, according to the Stewart-Figge theory, the metabolic independent acid-base variables include the strong ion difference ([SID]) and the total concentrations of weak acids (albumin and inorganic phosphate), which are not shown in the conventional Gamblegram. Thus, the Gamblegram in its current form is unsuitable for visualizing acid-base perturbations using the Stewart-Figge approach.

Long-acting muscarinic antagonists (LAMAs) are a class of inhalers that has recently been included as add-on therapy in the GINA guidelines, either in a single inhaler device with inhaled corticosteroids plus long-acting β2-agonists (ICS + LABA) (closed triple inhaler therapy) or in a separate one (open triple inhaler therapy). This review summarizes the existing evidence on the addition of LAMAs in patients with persistently uncontrolled asthma despite ICS + LABA treatment based on clinical efficacy in the reduction of asthma symptoms and exacerbations, the improvement in lung function, and its safety profile.

Article Synopsis
  • - Invasive pulmonary aspergillosis (IPA) is a significant concern for critically ill COVID-19 patients, but accurately measuring its prevalence and effects is complicated due to inconsistent symptoms and varying clinical practices.
  • - Diagnosing COVID-19-associated pulmonary aspergillosis (CAPA) relies on specific tests (like serum and BAL GM tests), as traditional methods often yield unreliable results, and bronchoscopy should be reserved for cases that would change treatment.
  • - Effective management of CAPA aims to enhance survival by preventing misdiagnosis and ensuring prompt antifungal treatment, considering factors like infection severity and potential drug interactions, though the best length of treatment is still debated.

Background: Laparoscopic cholecystectomy is the most common surgical procedure performed in the Western world. While it is performed with minimally invasive procedures, patients often complain of moderate to severe postoperative pain, and the role of the anesthesiologist for its effective management remains crucial. Modern anesthesiology practices have embraced trunk blocks which can contribute to perioperative, multimodal analgesia.

Acute lymphoblastic leukemia (ALL) represents the most common pediatric cancer accounting for about one-third of all malignancies in childhood. The differential diagnosis for a pediatric patient manifesting with joint pain and refusal to bear weight is wide and includes trauma, transient synovitis, septic arthritis, rheumatologic disorders, and malignancy. Overt complaints from the musculoskeletal system as the initial manifestation of ALL may present in up to 30% of cases with normal laboratory tests and without hepatosplenomegaly or lymphadenopathy, perplexing the establishment of a definite diagnosis.

Introduction: The role of comorbidities in determining health-related quality of life (HRQL) in obstructive sleep apnea (OSA) pa-tients has not been thoroughly investigated. Commonly used comorbidity tools, such as Charlson Comorbidity Index (CCI), have been designed with mortality as the outcome variable. A new tool, the Functional Comorbidity Index (FCI), has been especially developed to assess the effect of comorbidities on the "physical functioning" subscale of the Medical Outcomes Short Form-36 Health Survey (SF-36).

Chronic obstructive pulmonary disease (COPD) is a leading cause of morbidity and mortality worldwide accompanied by a substantial social and economic burden for the patient and the society. Poor sleep quality among COPD patients is frequently unnoticed and unaddressed by physicians and patients themselves, although it is a major source of further deterioration of these patients' quality of life. The aim of the present study was to record the quality of sleep in COPD patients among the Greek population and correlate these findings with various features of these patients, using the COPD and Asthma Sleep Impact Scale (CASIS).

Electronic Cigarettes and Asthma: What Do We Know So Far?

J Pers Med

July 2021

Clinic of Respiratory Failure, General Hospital of Thessaloniki Georgios Papanikolaou, Aristotle University of Thessaloniki, Leoforos Papanikolaou, 57010 Thessaloniki, Greece.

Article Synopsis
  • Electronic cigarettes (EC) are seen as alternatives to traditional tobacco cigarettes, but their health impacts, especially on asthma patients, are not well-understood.
  • * The review evaluates how ECs may contribute to asthma development, negatively impact lung function, worsen asthma symptoms, and increase inflammation in those who vape.
  • * Evidence indicates that asthma patients who use ECs face more severe symptoms and exacerbations, suggesting they should avoid using them altogether.

Article Synopsis
  • - The Stewart-Figge acid-base model is complex, so researchers created a simpler bedside version using mid-reference concentrations for calcium and magnesium and setting pH to 7.4.
  • - The new model features adjusted values for strong ion difference ([SIDa, adj]) and a "bicarbonate gap" ([BICgap]), with high accuracy for diagnosing various acid-base disorders in patients with respiratory issues.
  • - The study showed strong correlations and predictive accuracies for diagnosing conditions like strong ion acidosis and unmeasured ion acidosis using the new model, which performs well compared to traditional methods.

Secondary aortobronchial fistula (ABF) is an uncommon clinical entity associated with increased mortality. Most common presentation is hemoptysis, ranging from repetitive self-limiting episodes to massive hematemesis. Mediastinitis (if present) and excessive blood loss burden the unfavorable case of ABF, whereas stent-graft contamination and sepsis are the main concerns regarding endovascular therapy.

Purpose: To suggest a simplified method for strong ion gap ([SIG]) calculation.

Patients And Methods: To simplify [SIG] calculation, we used the following assumptions: (1) the major determinants of apparent strong ion difference ([SID]) are [Na], [K] and [Cl] (2) [Ca] and [Mg] do not contribute significantly to [SID] variation and can be replaced by their reference concentrations (3) physiologically relevant pH variation is at the order of 10 and therefore we can assume a standard value of 7.4.
