Stem fixation techniques in revision total knee arthroplasty: A systematic review and meta-analysis.

J Exp Orthop

January 2025

Department of Orthopaedic Surgery and Traumatology Città della Salute e della Scienza Turin Italy.

Purpose: This systematic review and meta-analysis aimed to compare the clinical and radiological outcomes of patients undergoing revision total knee arthroplasty (rTKA) using uncemented press-fit stems (hybrid fixation) versus cemented stems (cemented fixation). It is also examined whether cemented fixation offers any superiority over hybrid fixation regarding implant survival, clinical function, imaging analysis and complication rates.

Methods: Following the PRISMA guidelines, a systematic review and meta-analysis were conducted on five databases (Pubmed, Scopus, Embase, Medline and Cochrane).

Introduction: Castleman disease (CD) represents a spectrum of heterogeneous lymphoproliferative disorders sharing peculiar histopathological features, clinically subdivided into unicentric CD (UCD) and multicentric CD (MCD) and presenting with variable inflammatory symptoms. Interleukin (IL)-6 and other cytokines play a major role in mediating CD inflammatory manifestations. Although the local microenvironment seems to be among the major sources of hypercytokinemia, the precise cellular origin of IL-6 production in CD is still debated.

Cerebral Fat Embolism After Traumatic Long-Bone Fracture: A Case Report.

Clin Case Rep

December 2024

Department of Anaesthesia, Intensive Care and Emergency Policlinico Paolo Giaccone Palermo Italy.

We here describe a rare case of a 26-year-old male patient, presenting to our hospital after a motorcycle-car accident for the surgical management of long-bone fractures to the tibia and femur and shoulder dislocation and undergoing orthopedic surgery, who postoperatively developed seizure, coma, and respiratory failure due to a cerebral fat embolism. After some hours after the surgery, a loss of consciousness was described by caregivers, and the ICU team was alerted, and the patient was admitted to ICU. Intracranial hemorrhage and stroke were subsequently excluded and, finally, a whole-body computed tomography angiography scan and head magnetic resonance imaging showed lesions consistent with cerebral fat embolism.

  • SIRS after mitral transcatheter edge-to-edge repair is linked to worse clinical outcomes in patients with severe mitral regurgitation.
  • Forty-four out of 158 patients (27.9%) developed SIRS post-procedure, which was characterized by elevated white blood cell counts and fever.
  • The presence of SIRS significantly increased the risk of major cardiovascular events, including nonfatal heart attacks and deaths, indicating the need for closer monitoring in these patients.

  • * The Ne/Nc ratio varies due to ecological and evolutionary factors, influencing conservation strategies, and different types of effective population sizes must be considered to grasp their implications.
  • * Despite technological advancements in marine species documentation, predicting the Ne/Nc ratio remains challenging, highlighting the need for more research to effectively manage genetic diversity and population viability in these fish populations.

Freshwater jellyfish (= limnic medusa-budding hydrozoans, FWJ) are a small group of cnidarians found on all continents except Antarctica in temperate to tropical latitudes. Members of this group belong primarily to three genera: , , and . While and are typically restricted to the islands of Japan, Africa, and the Indian subcontinent, one species or potential species complex, , became globally invasive.

Objectives: To understand how best to further reduce the inappropriate use of pre-surgical androgen deprivation therapy (ADT), we investigated the determinants (influences) of ADT prescribing in urologists in two European countries using an established behavioural science approach. Additionally, we sought to understand how resource limitations caused by COVID-19 influenced this practice. Identification of key determinants, of undistributed and disrupted practice, will aid development of future strategies to reduce inappropriate ADT prescribing in current and future resource-limited settings.

Article Synopsis
  • Marine top predator populations, including a third of chondrichthyans, are declining, with the Mediterranean blue shark classified as critically endangered due to limited data on their population structure and connectivity.
  • A genetic study on 203 blue shark specimens identified subtle genetic differences both within the Mediterranean and between it and the Northeast Atlantic, suggesting demographic independence of Western and Eastern populations.
  • Despite limited reproductive interactions, the slight genetic differentiation hints at a recent shared history, indicating that Mediterranean blue sharks may act as a metapopulation influenced by local demographic changes and poor regulation of fishing practices.

Despite increasing knowledge about small extracellular vesicle (sEV) composition and functions in cell-cell communication, the mechanism behind their biogenesis remains unclear. Here, we reveal for the first time that sEV biogenesis and release into the microenvironment are tightly connected with another important organelle, Lipid Droplets (LDs). The correlation was observed in several human cancer cell lines as well as patient-derived colorectal cancer stem cells (CR-CSCs).

Melanism, the process of heavier melanin deposition, can interact with climate variation at both micro and macro scales, ultimately influencing color evolution in organisms. While the ecological processes regulating melanin production in relation to climate have been extensively studied, intraspecific variations of melanism are seldom considered. Such scientific gap hampers our understanding of how species adapt to rapidly changing climates.

Objectives: The aim of this study was to investigate the retinal morpho-functional characteristics of patients with neovascular wet age-related macular degeneration (nAMD) treated with intravitreal injection (IV) of aflibercept (AFL).

Methods: The study was conducted on 35 patients previously diagnosed with type 1 nAMD who received a fixed-dosing regimen of aflibercept injections over 12 months. The goal was to assess trends in visual abilities over time by measuring visual acuity (VA), contrast sensitivity (CS), visual evoked potentials (VEPs), and spectral domain-optical coherence tomography (SD-OCT).

Monocyte distribution width (MDW) kinetic for monitoring sepsis in intensive care unit.

Diagnosis (Berl)

November 2024

Department of Biomedicine, Neurosciences and Advanced Diagnostics, Institute of Clinical Biochemistry, Clinical Molecular Medicine, and Clinical Laboratory Medicine, 18998 University of Palermo Palermo, Italy.

Objectives: Monocyte distribution width (MDW) is a measure of monocyte anisocytosis. In this study, we assessed the role of MDW, in comparison to C-reactive protein (CRP), procalcitonin (PCT), and lactate, as a screening and prognostic biomarker of sepsis in intensive care unit (ICU) by longitudinally measuring it in the first 5 days of hospital stay.

Methods: We considered all consecutive patients admitted to the ICU.

  • This study compares two surgical methods, Hartmann's procedure (HP) and resection with primary anastomosis (RPA), for treating acute left-sided colonic emergencies among 1215 patients from 204 centers globally.
  • Results showed that while HP was the more common treatment (57.3%), RPA was favored for younger patients with fewer health issues and those needing surgery sooner.
  • The study concluded that although HP is still widely used, RPA might be the better option, emphasizing the importance of patient characteristics and surgeon experience in determining treatment choice.

Article Synopsis
  • Ocean acidification (OA) impacts fish species by affecting their physiology, behavior, and survival, but some species, like the anemone goby, show rapid acclimation and thrive in these conditions.
  • Researchers studied wild anemone gobies in a natural CO seep on Vulcano Island, discovering changes in gene expression related to ion transport, stress response, and metabolism, which help them adapt to high CO environments.
  • The findings suggest that these gobies possess molecular mechanisms that allow them to cope with oxidative stress and maintain physiological balance, potentially giving them an edge in their acidic habitat.
