Aims: The use of loop diuretics in pulmonary arterial hypertension (PAH) is less frequent compared with heart failure. The clinical and prognostic characteristics of PAH patients according to loop diuretic use remain unexplored. In this study, we retrospectively analysed the characteristics and survival of PAH patients requiring different doses of loop diuretics.

Background: ChatGPT has recently emerged as a novel resource for patients' disease-specific inquiries. There is, however, limited evidence assessing the quality of the information. We evaluated the accuracy and quality of the ChatGPT's responses on male lower urinary tract symptoms (LUTS) suggestive of benign prostate enlargement (BPE) when compared to two reference resources.

Article Synopsis
  • * Conducted across 34 centers globally from 2000 to 2020, the study included over 2,000 adult patients split into different ECMO duration groups, revealing higher complications and mortality rates associated with longer ECMO durations.
  • * The findings suggest that while most in-hospital complications increase with ECMO duration, post-discharge survival rates remain similar across groups, with specific risk factors like age and pre-existing conditions affecting long-term survival for those on ECMO longer than 7 days.

Article Synopsis
  • - The study examined fertility preservation methods in 414 female lymphoma patients aged 18-40 diagnosed between 2010 and 2018, with most common cancer types being Hodgkin Lymphoma (74%) and various others.
  • - Majority of the patients (71%) received ABVD as the first-line treatment, while fertility preservation strategies included GnRHa (78%), oral contraceptives, and cryopreservation of oocytes and ovarian tissue.
  • - After treatment, 70% of women had regular menstrual cycles, 8% experienced premature ovarian failure, and there were 43 pregnancies observed during a median follow-up of 5 years, with age and treatment intensity being significant factors for amenorrhea and POF.

Introduction: Hypertension is the main risk factor for cardiovascular diseases (CVD). Notably, only about half of hypertensive patients manage to achieve the recommended blood pressure (BP) control. Main reasons for the persistence of uncontrolled BP during treatment are lack of compliance on the patients' side, and therapeutic inertia on physicians' side.

Article Synopsis
  • The study evaluated different models/scores to identify hemodynamically stable patients with acute pulmonary embolism (PE) who are at risk of death or clinical deterioration.
  • Among the 5036 patients analyzed, most models had positive predictive values below 10%, but negative predictive values were over 98% for most, indicating they are good at ruling out the risk.
  • TELOS and NEWS2 showed the best performance for predicting in-hospital death and deterioration, while TELOS stood out with the highest accuracy for identifying at-risk patients.

Objectives: We investigated the occurrence and relative contribution of relapse-associated worsening (RAW) and progression independent of relapse activity (PIRA) to confirmed disability accrual (CDA) and transition to secondary progression (SP) in relapsing multiple sclerosis (MS).

Methods: Relapsing-onset MS patients with follow-up > / = 5 years (16,130) were extracted from the Italian MS Registry. CDA was a 6-month confirmed increase in Expanded Disability Status Scale (EDSS) score.

Background And Aims: We aimed to characterize the epidemiologic and comorbidities profiles of patients with chronic Hepatitis D (CHD) followed in clinical practice in Italy and explored their interferon (IFN) eligibility.

Methods: This was a cross-sectional study of the PITER cohort consisting of consecutive HBsAg-positive patients from 59 centers over the period 2019-2023. Multivariable analysis was performed by logistic regression model.

The treatment of hepatitis C virus (HCV) with direct-acting antivirals (DAA) leads to high sustained virological response (SVR) rates, but hepatocellular carcinoma (HCC) risk persists in people with advanced liver disease even after SVR. We weighted the HCC risk in people with cirrhosis achieving HCV eradication through DAA treatment and compared it with untreated participants in the multicenter prospective Italian Platform for the Study of Viral Hepatitis Therapies (PITER) cohort. Propensity matching with inverse probability weighting was used to compare DAA-treated and untreated HCV-infected participants with liver cirrhosis.

Over the past decade, remote monitoring (RM) has become an increasingly popular way to improve healthcare and health outcomes. Modern cardiac implantable electronic devices (CIEDs) are capable of recording an increasing amount of data related to CIED function, arrhythmias, physiological status and hemodynamic parameters, providing in-depth and updated information on patient cardiovascular function. The extensive use of RM for patients with CIED allows for early diagnosis and rapid assessment of relevant issues, both clinical and technical, as well as replacing outpatient follow-up improving overall management without compromise safety.

Development and Validation of a Scoring System to Predict Response to Obeticholic Acid in Primary Biliary Cholangitis.

Clin Gastroenterol Hepatol

October 2024

Division of Gastroenterology, Centre for Autoimmune Liver Diseases, Department of Medicine and Surgery, University of Milano-Bicocca, European Reference Network on Hepatological Diseases, San Gerardo Hospital, Monza, Italy. Electronic address:

Article Synopsis
  • The study focused on creating a scoring system called the OCA response score (ORS) to predict how individuals with primary biliary cholangitis (PBC) will respond to the treatment using obeticholic acid (OCA).
  • Data were collected from two large cohorts in Italy to derive and validate the score, which includes various clinical factors both before and after six months of treatment.
  • The scoring system demonstrated good predictive ability for treatment response, which could help healthcare providers customize therapies for patients with PBC more effectively.

Background: There is a growing need for percutaneous coronary intervention (PCI) to be performed within the same transcatheter aortic valve implantation (TAVI) procedure. In such cases, cangrelor, a fast-acting intravenous P2Y12-inhibitor with a short offset, is potential clinical utility to minimize bleeding and vascular complications during large-bore arterial access (LBAA) as well as the thrombotic risk associated with concomitant PCI.

Case Summary: We report two cases of TAVI with an indication to concomitant, high-risk PCI.

Features and outcomes of female and male patients requiring postcardiotomy extracorporeal life support.

J Thorac Cardiovasc Surg

December 2024

Cardiovascular Research Institute Maastricht, University of Maastricht, Maastricht, The Netherlands; Cardio-Thoracic Surgery Department, Maastricht University Medical Centre, Maastricht, The Netherlands.

Objectives: Although cardiogenic shock requiring extracorporeal life support after cardiac surgery is associated with high mortality, the impact of sex on outcomes of postcardiotomy extracorporeal life support remains unclear with conflicting results in the literature. We compare patient characteristics, in-hospital outcomes, and overall survival between females and males requiring postcardiotomy extracorporeal life support.

Methods: This retrospective, multicenter (34 centers), observational study included adults requiring postcardiotomy extracorporeal life support between 2000 and 2020.

Off-label use of Lumen-apposing metal stents for treatment of short benign biliary strictures.

Dig Liver Dis

October 2024

Digestive and Interventional Endoscopy Unit, Niguarda Hospital, Piazza dell'Ospedale Maggiore 3, 20161, Milan, Italy.

Article Synopsis
  • Endoscopic stenting, particularly using lumen-apposing metal stents (LAMS), is a key treatment for benign biliary strictures, which can recur or cause stent migration.
  • A study involving 70 patients showed a 100% technical success rate and an 85.7% clinical success rate for treating these strictures with bi-flanged LAMS over six years.
  • While 17.1% of patients experienced adverse events like stent migration, LAMS demonstrated effectiveness, especially in post-surgical cases, suggesting it is a safe option for managing short benign biliary strictures.
