A proposal for incorporating health level seven (HL7) vocabulary in the UMLS Metathesaurus.

Proc AMIA Symp

Intermountain Health Care, Salt Lake City, UT, USA.

Published: March 1999

The HL7 Vocabulary Technical Committee (TC) was organized to select and maintain the vocabulary used in HL7 messages. The goal is to make implementations of the Version 3 HL7 Standard more plug-and-play compatible. In order to make the vocabulary readily accessible to the public, HL7 is collaborating with the U.S. National Library of Medicine (NLM) to include HL7 vocabulary in the Unified Medical Language System (UMLS) Metathesaurus. This article describes a proposal for how HL7 data elements and coded values can be represented accurately in the relational tables of the UMLS Metathesaurus.

Article Synopsis
  • The text discusses the importance of both structural and semantic standards for medical data interoperability, focusing on FHIR for structure and SNOMED CT for semantic meaning.
  • It introduces a solution for storing postcoordinated expressions (PCEs) by integrating them with the FHIR data model, allowing effective medical data interchange.
  • The proposed method involves breaking down PCEs into components aligned with FHIR resources and using tools like OWL and reasoning processes to identify the best-matching pre-coordinated SNOMED CT concepts.

We are creating a synergy among European Health Data Space projects (e.g., IDERHA, EUCAIM, ASCAPE, iHELP, Bigpicture, and HealthData@EU pilot project) via health standards usage thanks to the HSBOOSTER EU Project since they are involved or using standards, and/or designing health ontologies.

This paper describes the development of Health Level Seven Fast Healthcare Interoperability Resource (FHIR) profiles for pathology reports integrated with whole slide images and clinical data to create a pathology research database. A report template was designed to collect structured reports, enabling pathologists to select structured terms based on a checklist, allowing for the standardization of terms used to describe tumor features. We gathered and analyzed 190 non-small-cell lung cancer pathology reports in free text format, which were then structured by mapping the itemized vocabulary to FHIR observation resources, using international standard terminologies, such as the International Classification of Diseases, LOINC, and SNOMED CT.

The COVID-19 pandemic has spurred large-scale, interinstitutional research efforts. To enable these efforts, researchers must agree on data set definitions that not only cover all elements relevant to the respective medical specialty but also are syntactically and semantically interoperable. Therefore, the German Corona Consensus (GECCO) data set was developed as a harmonized, interoperable collection of the most relevant data elements for COVID-19-related patient research.

Interoperability in healthcare cannot be achieved without mapping local data to standardized terminology. In this paper, we investigate the performance of different approaches for implementing HL7 FHIR Terminology Module operations using a benchmarking methodology, to gather evidence on the benefits and pitfalls of these methods in terms of performance from the point-of-view of a terminology client. The approaches perform very differently, while having a local client-side cache for all operations is of supreme importance.
