We conducted a retrospective study of six patients presenting a parapharyngeal tumor and an analysis of data of the literature and recall classification of these tumors and their anatomo-clinics characters, therapeutic and diagnostic problems highlighting. These tumors are rare and have various origins, their treatment is almost always surgical by cervical approach which can be undertaken only after a rigorous exploration, systematically associating computed tomography scan and magnetic resonance imaging. Transoral resection is strongly advised against as it is potentially harmful.

This case report describes a 4.5-year-old girl diagnosed with a rare Undifferentiated Small Round Cell Sarcoma (USRCS) originating in the parapharyngeal space with multiple lung metastases. Diagnostic workups, including imaging, immunohistochemistry, and genetic sequencing, identified the tumor as an unclassified subtype of USRCS.

Pleomorphic adenoma of the parapharyngeal space is a rare benign tumor, representing less than 1% of tumors of the parapharyngeal space. Considering the volume of the tumor, the complexity of the space and the potential difficulties of excision, an imaging work-up including at least computed tomography (CT) and/or magnetic resonance imaging (MRI), and possibly analysis of the vascular axes (angio-MRI, arteriography) is essential. Treatment of these tumors is essentially surgical.

Diagnosis of lesions of the parapharyngeal space (PPS) often poses a diagnostic and therapeutic challenge due to its deep location. As a result of the topographical relationship to nearby neck spaces, a very precise differential diagnosis is possible based on imaging criteria. When in doubt, imaging-guided - usually CT-guided - biopsy and even drainage remain options.
