Article Synopsis
  • - Group III pulmonary hypertension (PH) is common in patients with hypersensitivity pneumonitis (HSP), but this case presents a rare instance of Group I PH in a patient with HSP.
  • - An elderly veterinarian with non-fibrotic HSP experienced worsening shortness of breath despite treatment with prednisone, prompting further examination and a right heart catheterization.
  • - Following the diagnosis of PH with vasoreactivity, nifedipine was prescribed, leading to significant symptom improvement and complete resolution of PH over a 9-month period, emphasizing the need for early diagnosis and treatment.

Background: We assessed the performance of Electromagnetic navigational bronchoscopy (ENB) as a standalone diagnostic technique and the performance of different sampling tools used during the procedure.

Methods: We recruited 160 consecutive patients who underwent ENB for peripheral lung lesions (PLL) at a tertiary care centre. The diagnostic performance of ENB and sampling tools was assessed using a logistic regression model and a ROC-curve in which the dependent variable was diagnostic success.

Article Synopsis
  • Transbronchial lung cryobiopsy (TBLC) is effective for diagnosing diffuse parenchymal lung diseases, with the balloon occlusion method recommended to manage bleeding during the procedure.
  • A study reviewed 20 patients who underwent TBLC using a modified balloon technique, focusing on the use of a 6-Fr balloon to minimize displacement compared to a 4-Fr balloon.
  • Results showed that successful balloon occlusion was achieved in all patients, indicating that this modified method is a viable option for preventing bleeding during TBLC.

Pneumothorax (Pnx) is the presence of air or gas in the pleural space which hinders the lungs to function appropriately. Pneumothorax that presents in the absence of any underlying aetiology is called primary spontaneous pneumothorax (PSP) and on the other hand, it may present as a complication of underlying lung disease which is known as secondary spontaneous pneumothorax (SSP). Iatrogenic Pneumothorax (IP) is a type of SSP and is caused by medical interventional procedures which include transthoracic needle biopsy (24%), sub clavicular catheterization (22%), thoracocentesis (20%), transbronchial biopsy (10%), pleural biopsy (8%) and positive pressure ventilation (7%).
