Errors enacted during endoscopic surgery--a human reliability analysis.

The aim of the study was to document the nature and incidence of surgical errors enacted during laparoscopic surgery in order to direct future research and surgical training. A modified Human Reliability Analysis (HRA) approach, based on direct observation, was adopted to categorise and record errors encountered during the practice of laparoscopic cholecystectomy. This study confirmed the applicability and usefulness of an observational methodology in the assessment of human error in endoscopic surgical performance. The study identified aspects of the design and usage of instruments, surgical training and the differences between tasks which needed further directed research in order to identify underlying performance shaping factors (PSFs) and so reduce error rates.

Importance: The current research aimed to conduct a detailed analysis of intraoperative surgical performance, short-term outcomes, identify and categorize technical errors, and hazard-zones enacted during total gastrectomy performed robotically and laparoscopically by surgeons. Prospective research is needed to determine whether the technical advantages of robotic surgery translate to patient outcomes.

Objective: Identify and process risk areas in robot-assisted total gastrectomy (RTG) and laparoscopic total gastrectomy (LTG) to get the best patient results.

The derivation of chromium (Cr) ecological risk thresholds in soils remains limited, despite their importance as measurement standards and indicators for enacting soil protection policies. In this study, toxicity of Cr in soil to different species was tested based on Log-Logistic dose-effect relationship. On this basis, combined with Cr toxicity measurement data in literature, the ecological risk threshold HC for protecting 95% species safety in soils with different properties was obtained by fitting species sensitivity distribution curve (SSD).
