Purpose: We examined a patient with acute posterior multifocal placoid pigment epitheliopathy and bilateral central retinal vein occlusion.

Method: Case report. A 28-year-old woman presented with the typical findings of acute posterior multifocal placoid pigment epitheliopathy. One month after presentation, the patient developed bilateral central retinal vein occlusion.

Result: Four months after presentation, resolution of the bilateral central retinal vein occlusion resulted in a corrected visual acuity of RE, 20/25 and LE, 3/400.

Conclusion: Vasculitis, sometimes associated with acute posterior multifocal placoid pigment epitheliopathy, can affect the ciliary vessels that supply the optic disk. This may result in axoplasmic stasis and compression of the central retinal vein, leading to impending or complete central retinal vein occlusion.
