A 20-year-old man was referred to our hospital with a complaint of asymptomatic macrohematuria. A diagnosis of a right renal tumor was made after several radiographic examinations and right radical nephrectomy was performed. Histopathologically the tumor was a congenital mesoblastic nephroma. Congenital mesoblastic nephroma is a relatively rare renal tumor predominantly of childhood. Occurrence in adults is exceedingly rare and 18 cases have been reported to date.

Approximately 20% of paediatric and adolescent/young adult patients with renal tumours are diagnosed with non-Wilms tumour, a broad heterogeneous group of tumours that includes clear-cell sarcoma of the kidney, congenital mesoblastic nephroma, malignant rhabdoid tumour of the kidney, renal-cell carcinoma, renal medullary carcinoma and other rare histologies. The differential diagnosis of these tumours dates back many decades, when these pathologies were identified initially through clinicopathological observation of entities with outcomes that diverged from Wilms tumour, corroborated with immunohistochemistry and molecular cytogenetics and, subsequently, through next-generation sequencing. These advances enabled near-definitive recognition of different tumours and risk stratification of patients.

To report the cosmetic, clinical, and visual outcomes of a combined surgical approach for treating a corneal/limbal dermoid using excision and a three-layered amniotic membrane graft with fibrin glue. An 18-year-old female presented with impaired vision and ocular discomfort caused by a prominent dome-shaped limbal congenital dermoid on the inferotemporal cornea, resulting in a significant aesthetic concern. A full assessment, including refraction, best-corrected visual acuity (BCVA), corneal topography, aberrometry and anterior segment OCT (AS-OCT) was conducted to plan the surgical approach.

Background: Congenital mesoblastic nephroma represents 3-10% of all pediatric renal tumors. With the advancement of ultrasound diagnostics and magnetic resonance imaging, the diagnosis of this renal neoplasm is increasingly being established prenatally and at birth. It usually presents as a benign tumor, but it can severely affect pregnancy outcomes, contributing to perinatal morbidity and mortality.

Paediatric renal tumours: an update on challenges and recent developments.

Virchows Arch

January 2025

Histology Laboratory, Children's Health Ireland, Dublin, Ireland.

Paediatric renal tumours include a broad range of neoplasms which largely differ, but also overlap to a smaller extent, with adult kidney cancer. These include the embryonal tumour nephroblastoma, which accounts for the majority of cases of kidney cancer in the first decade of life and, despite boasting a cure in ~ 90% cases, still presents clinical challenges in a small proportion of cases. A variety of less common mesenchymal tumours, including the mostly indolent congenital mesoblastic nephroma, clear cell sarcoma of kidney which continues to be associated with poor outcomes for higher stage disease, and the typically lethal malignant rhabdoid tumour, form the bulk of the remaining presentations in the first decade.

Characterization of malignant kidney tumors in childhood by F-FDG PET/CT.

World J Urol

December 2024

Department of Nuclear Medicine, Xinhua Hospital, Shanghai Jiao Tong University School of Medicine, Kongjiang Road 1665, Yangpu District, Shanghai, 200092, China.

Article Synopsis
  • The study examines how semiquantitative parameters from F-FDG PET/CT can help characterize pediatric kidney tumors, focusing on 54 children with confirmed malignant tumors.
  • Results show that while PET parameters like SUVmax, MTV, and TLG detected all kidney tumors, only clear cell sarcoma had significantly lower FDG uptake compared to Wilms tumor.
  • The findings highlight that specific PET metrics are closely linked to disease stage, with TLG being the best predictor for advanced stages, which could help in understanding genetic factors in Wilms tumor.
