Paget's disease is a condition marked by abnormal bone remodeling, involving both excessive bone formation and destruction, predominantly in the elderly. Pagetic vertebral ankylosis is a rare manifestation, often associated with Paget's disease, ankylosing spondylitis, or diffuse idiopathic skeletal hyperostosis. This form of acquired vertebral ankylosis is uncommon and occurs in cases with bone-bridging syndesmophytes or osteophytes.

Background: Paget's disease of bone (PDB) is a common bone metabolic pathology in older adults, characterized by mixed osteolytic, osteoblastic, and quiescent periods. Surgical guidelines for PDB involving the spine are not well-defined and are reserved for cases refractory to medical treatments, typically bisphosphonates like zoledronic acid. This case study describes a 52-year-old male with PDB who presented with rapidly progressing myelopathy symptoms refractory to standard medical treatment, warranting surgical decompression.

Palpation of the seventh cervical vertebra (C7) is an important landmark for counting vertebrae for vertebral spine surgical instrumentation. However, studies have shown that the spinous process of C7 displays an anatomical deviation among individuals, which may mislead a surgeon who is not aware of this, and there have been no such studies among southwest Nigerians. The present study aimed to examine the incidence of bifidity in the C7 spinous process and their variation among 48 subjects with the aid of a three-dimensional (3D) computed tomography.
