Use of Narrow Band Imaging to Guide Endobronchial Biopsy for Suspected Sarcoidosis.

J Bronchology Interv Pulmonol

April 2025

Department of Medicine, Division of Respirology, University of Calgary, Calgary, AB.

Background: Diagnosis of sarcoidosis often involves endobronchial biopsy (EBB), but studies have shown varying yields for EBB in suspected sarcoidosis, partly due to differences in identifying abnormal mucosa under white light (WL). Narrow band imaging (NBI) may assist in the visualization of abnormal mucosa, but its role in sarcoidosis remains to be characterized.

Methods: Individuals referred for suspected sarcoidosis were considered for enrollment.

Tunneled Pleural Catheters: An Effective Nonsurgical Alternative for Nonexpandable Lung in Chronic Pleural Infection.

J Bronchology Interv Pulmonol

January 2025

Division of Thoracic Surgery and Interventional Pulmonology, Beth Israel Deaconess Medical Center, Harvard Medical School.

Background: Open window thoracostomy (OTW) is the standard of care for debilitated patients with chronic pleural infection and nonexpandable lungs (NEL) who are not candidates for major surgical intervention. Tunneled pleural catheters (TPC) offer tremendous treatment potential in this setting based on their efficacy in malignant pleural effusion and NEL. We aim to assess the efficacy, safety, and health care utilization of TPC in this setting.

Article Synopsis
  • The American Association of Bronchology and Interventional Pulmonology aims to keep clinicians informed with brief, current reviews on essential interventional pulmonology topics, starting with malignant pleural disease.
  • The article consists of three parts that update readers on malignant pleural effusion: diagnosis through imaging and fluid biomarkers, management strategies including multimodal approaches and targeted therapies, and details on pleural mesothelioma.
  • These reviews are part of the Essential Knowledge series presented at the 2023 AABIP Annual Conference, and recorded lectures are available for access on the AABIP website.

Background: Bronchoscopic lung volume reduction (BLVR) is a minimally invasive procedure used to reduce shortness of breath and improve functionality in some patients with emphysema. While BLVR is often effective for improving dyspnea by causing target lobe atelectasis, the treatment effect can sometimes be lost. This study reviews the incidence of revision bronchoscopies in patients who lost or never achieved target lobe atelectasis following BLVR.

Ultrasound (US)-guided fine-needle aspiration (FNA) is a minimally invasive procedure that can be performed by an in-training pulmonologist. This is a retrospective study of 73 patients who underwent US-guided FNA by pulmonologist and interventional radiologist. Comparable diagnostic yield was found in this study (88.
