Of 122 patients with single-chamber implantable cardioverter-defibrillators (ICDs) reviewed retrospectively, 35 had traditional indications, 14 had other indications, and 18 had ICD-specific indications for dual-chamber pacing therapy. Thus, 67 patients (55%) were potential candidates for dual-chamber pacing, which has only recently become available combined with ICD therapy.

Reel's syndrome (RS) is an unusual cause of pacemaker lead dislodgement. We present the case of a 59-year-old female patient with Down syndrome (DS) implanted with a dual-chamber endovascular pacemaker due to symptomatic sinus node disfunction, reporting several syncopal episodes in last days and showing abnormal electrical parameters at the 2-months follow-up due to RS. The malfunctioning device was removed and an endocardial leadless pacing system was implanted.

We present the case of an 80-year-old female with acute pulmonary edema and a dual chamber pacemaker with intermittent short AV delays in the surface ECG after blocked premature atrial contractions (PACs). The behavior was consistent with the programmed Window of Atrial Rate Acceleration Detection (WARAD) and did not require further parameter modifications. As most cardiologists and emergency department physicians are not familiar with brand-specific algorithms, we believe that this case report will make these noncompetitive atrial pacing algorithms more accessible to non-cardiologists.
