Schiotz tonometry for glaucoma: are there simple alternatives?

In the period from October 1991 until August 1992, 864 eye-patients in a rural area in south-east Nigeria were examined in order to find simple parameters for the screening of glaucoma simplex. Schiotz tonometry was used as the gold standard. Investigation of the pupil-reaction with a torch, estimation of the pressure of the (closed) eye by digital palpation, and estimations of the cup/disc ratio by ophthalmoscopy can be used as an alternative.

Glaucoma is a heterogeneous group of optic neuropathies characterized by a progressive loss of retinal ganglion cells (RGCs) with corresponding visual field defects, and one of the main risk factors is elevated intraocular pressure (IOP). Furthermore, colonoscopy procedures require insufflation of the colon lumen with gases which can increase intraabdominal pressure (IAP) and ends with an elevation of IOP. Glaucoma is an infrequent complication due to colonoscopy; in this case, a 63 years-old woman was diagnosed with glaucoma after a colonoscopy procedure.

Tono-pen and Schiotz tonometer measurements in gas-filled eyes.

Purpose: This study aims to assess the difference between Tono-Pen and Schiotz tonometer measurements in gas-filled eyes and to create a nomogram and equation which can be used to estimate actual intraocular pressure in order to provide a safe IOP level at the end of the surgery.

Methods: Twenty-two eyes that underwent pars plana vitrectomy were included in the study. Perioperative Tono-Pen and Schiotz tonometer measurements were performed when the eyes were filled with air in the setting of certain vitrectomy infusion pressure levels.

Intraocular pressure (IOP) is maintained through complex and interrelated systems which control aqueous production and drainage, and it has been suggested that scleral lens (SL) wear may disrupt these vital homeostatic processes. This review provides an overview of anatomical and physiological processes that control IOP, identifies potential effects of SLs on these regulatory mechanisms, and examines studies that have attempted to quantify the effect of SLs on IOP. Lack of access to the cornea during SL wear makes accurate assessment of IOP challenging; therefore, a range of different assessment techniques and instruments have been employed to quantify IOP during and following SL wear.
