To determine the effect of unilateral posteroventral pallidotomy (PVP) on latency and duration of response to L-dopa ipsilateral and contralateral to the side of the lesion, six severe fluctuating Hoehn-Yahr IV patients with Parkinson's disease were evaluated 2 days before and after unilateral PVP. After an overnight drug holiday in a fasting state, patients were challenged with a single dose of 200/50 mg of L-dopa/ carbidopa. Sequential tapping tests every 5 minutes in both hands were used to measure changes in motor status. Duration of L-dopa effect was significantly longer after PVP (p < 0.03). Mean latency of the effect improved by 50% without reaching statistical significance. Despite a clear, asymmetric improvement of cardinal Parkinson's disease symptoms after unilateral PVP, changes in the dose-response L-dopa profile occurred symmetrically, suggesting that mechanisms underlying the two effects are distinct.

Background: Autism spectrum disorder (ASD) is a heterogeneous neurobiological condition characterized by behavioral problems and delayed neurodevelopment. Although transcranial magnetic stimulation (TMS) has been proposed as an alternative treatment for patients with ASD because of its promising benefits in reducing repetitive behaviors and enhancing executive functions, the use of high-intensity pulses (Hi-TMS) appears to be related to the side effects of the therapy. Low-intensity TMS (Li-TMS) has been partially investigated, but it may have clinical effects on ASD and simultaneously increase treatment safety.

Background And Objectives: Telemedicine has become a mainstay of ALS clinical care, but there is currently no standardized approach for assessing and tracking changes to the neurologic examination in this format. The goal of this study was to create a standardized telemedicine-based motor examination scale to objectively and reliably track ALS progression and use Rasch methodology to validate the scale and improve its psychometric properties.
Methods: A draft telemedicine examination scale with 25 items assessing movement in the bulbar muscles, neck, trunk, and extremities was created by an ALS expert panel, incorporating input from patient advisors.

Aside from typical motor symptoms, impulse control issues related to engaging in actions or decision-making can manifest in Parkinson's disease (PD). The lack of direct comparisons between impulsive action and impulsive choice domains hinders a comprehensive understanding of impaired impulse control in this population. Therefore, the current review integrates findings across behavioural measures of impulsive action and choice in PD samples, both on and off dopaminergic medication.
