Spontaneous annular detachment of the uterine cervix during labour is a rarely encountered obstetric complication that may pass unrecognized. The aetiological factors involved in its development remain incompletely understood. In this report, a case of a 40-year-old multiparous woman in whom annular detachment of the cervix passed completely unnoticed in the course of a spontaneous vaginal delivery to become evident, for the first time, during subsequent delivery is presented. Intriguingly, pelvic examination revealed an absent portio vaginalis of the cervix; the external cervical os was replaced by a small fibrosed opening. Fertility was well preserved, despite marked cervical fibrosis. A transverse lower segment caesarean section with tubal sterilization was carried out. The postoperative course was uneventful and likewise, the course of the puerperium was benign.

When a simply supported thin disk is indented by a centrally applied point force, it buckles out-of-plane to form a shape dominated by two conical portions: a uniform region indenting against the support, interrupted by a smaller elevated portion detached from the support, altogether known as a "developable cone" or d-Cone. If a central circular region of the disk is clamped instead, then the buckling complexion changes markedly: The indenting region is interspersed with several detached and elevated cones, now "truncated," where their number depends on the clamping extent as well as the radius of the circular simple support. Studies of d-Cone kinematics often consider its shape as an analogous vertex, which forms by folding along hinge lines separating triangular facets.

Aquaporin-0 (AQP0) tetramers form square arrays in lens membranes through a yet unknown mechanism, but lens membranes are enriched in sphingomyelin and cholesterol. Here, we determined electron crystallographic structures of AQP0 in sphingomyelin/cholesterol membranes and performed molecular dynamics (MD) simulations to establish that the observed cholesterol positions represent those seen around an isolated AQP0 tetramer and that the AQP0 tetramer largely defines the location and orientation of most of its associated cholesterol molecules. At a high concentration, cholesterol increases the hydrophobic thickness of the annular lipid shell around AQP0 tetramers, which may thus cluster to mitigate the resulting hydrophobic mismatch.

Background: Two subtypes of atrial functional mitral regurgitation (AFMR) have been described, one is characterized by Carpentier type I and the other by Carpentier type IIIb leaflet motion.
Objectives: The authors sought to analyze echocardiographic characteristics and outcomes of AFMR subtypes undergoing mitral valve transcatheter edge-to-edge repair (M-TEER).
Methods: Of 1,047 consecutive patients who underwent M-TEER, the authors identified those with isolated mitral annulus dilation (Carpentier I), termed AFMR-IAD, and those with atriogenic hamstringing characterized by restricted posterior mitral leaflet motion (Carpentier IIIb), termed AFMR-AH.
