Obstructed hemivagina with ipsilateral renal anomaly (OHVIRA) is a rare congenital genitourinary defect with a triad of unilateral vaginal obstruction, uterine anomaly and ipsilateral renal agenesis. This paper reports an unusual presentation of OHVIRA, with our patient experiencing severe abdominal pain from a left tubo-ovarian abscess that is contralateral to the side of the vaginal outflow obstruction. Another reportable finding is our patient's rare association of a trifid ureter that fuses distally before inserting ectopically in her vaginal canal.

Trifid ureter is a rare malformation with less than one hundred cases reported in the literature. However, like ectopic ureter, it is often asymptomatic and its real incidence is uncertain. The authors report a unique case of ectopic ureter draining into the prostatic urethra associated with trifid ureter, discovered during adulthood in a context of symptomatic ureteropelvic reflux.

A rare congenital malformation consisting of ectopic opening of the vas deferens into the fused segment of a refluxing triplicated ureter (trifid ureter) in an azoospermic man is described. Sperm were found in urine voided after ejaculation. Artificial insemination with sperm recovered from alkalinized urine resulted in pregnancy.
