Pneumoperitoneum as a complication of barotrauma is a rare condition due to mechanical ventilation. The diagnostic dilemma often leads to perform an exploratory laparotomy, with the suspect of perforation of a hollow viscus, that makes worse the prognosis of these patients. We describe three patients who developed pneumoperitoneum, secondary to barotrauma. In the last one the use of laparoscopy allowed us to avoid an useless laparotomy.

Emphysematous pyelonephritis is a necrotizing infection of the renal parenchyma by gas-forming organisms, with a risk of the gas extending into peri-nephric or para-renal spaces and in advanced cases, the involvement may be extensive and bilateral. It is a common complication in patients with long-term diabetes, primarily caused by Gram-negative organisms or, in some cases, anaerobes. The diagnosis of emphysematous pyelonephritis is made by clinical features and confirmed by computed tomography.

Meckel's diverticulum, a congenital anomaly of the omphalomesenteric duct, is a rare cause of acute abdomen and poses significant diagnostic challenges due to its variable presentation and complications such as perforation. We present the case of a 19-year-old male with a 24-hour history of right iliac fossa pain progressing to generalized abdominal tenderness. Laboratory results showed leukocytosis and elevated C-reactive protein, while CT imaging suggested pneumoperitoneum and an inflamed Meckel diverticulum.
