Lumbar spinal neurinoma presenting with nocturnal pain. A case report.

Clin Imaging

Department of Clinical Radiology, University of Muenster, Germany.

Published: February 1998

Neurinomas are the most common intradural extramedullary tumors of the spine. Together with meningeoma they account for 80% to 90% of all tumors in this compartment. Radicular pain is the initial symptom in the majority of cases while motor disorders are less common and occur later in the course of the disease. We report a patient with spinal neurinoma and calf pain as the only presenting complaint. The differential diagnosis in this case required an interdisciplinary approach.

Background: Spinal schwannomas presenting with an intraspinal hematoma or subarachnoid hemorrhage are extremely rare, and patients often have severe spinal cord compression symptoms. However, the mechanism underlying the bleeding remains unclear.

Case Presentation: We present the case of a 53-year-old Chinese female diagnosed with a T12 schwannoma accompanied by an intratumoral hematoma.

Schwannomas of C1: A Novel Classification and Description of Growth Patterns Based on a Personal Series and Review of Literature.

Neurol India

November 2024

Department of Neurosurgery, Kauvery Institute of Brain and Spine, Kauvery Hospital, Chennai, Tamil Nadu, India.

Background: Schwannomas of C1 are rare tumors. Because of the wide variation in their anatomy, and presentation and their rarity, there is no proper understanding of their pathological anatomy nor their optimal management.

Methods: A retrospective study of the C1 schwannomas operated by the author between 2000 and 2020 and a PubMed-based search of English literature were done to analyze data on the location and extent of the lesions, surgical approaches used, and operative outcomes.

An experience with two rare complications during surgery of vestibular schwannomas (VSs) is presented, and measures to avoid and manage the complications are discussed.Case A: Spinal cord ischemia in semi-sitting position: A 47-year-old with a giant vestibular schwannoma (VS) underwent surgery through a retrosigmoid approach in the semi-sitting position. The intraoperative phase was uneventful, except for an episode of moderate hypotension.

Article Synopsis
  • Schwannomas are benign tumors typically found near spinal nerve roots, but large cases with cystic degeneration are difficult to diagnose before surgery and are not well-researched in medical literature.
  • A case is presented of a 28-year-old man with a giant schwannoma causing intermittent pain in his left thigh; MRI revealed a complex cystic lesion extending from L4 to S1.
  • Successful surgical removal of the tumor led to symptom resolution, highlighting the need for accurate diagnosis through MRI and histopathology, and emphasizing that complete surgical excision is the preferred treatment approach.

Neurofibromatosis type 2 (NF-2) is a dominantly inherited genetic disorder that results from variants in the tumor suppressor gene, neurofibromin 2 (NF2). Here, we report the generation of a conditional zebrafish model of neurofibromatosis established by inducible genetic knockout of nf2a/b, the zebrafish homologs of human NF2. Analysis of nf2a and nf2b expression revealed ubiquitous expression of nf2b in the early embryo, with overlapping expression in the neural crest and its derivatives and in the cranial mesenchyme.
