To assess the value of surgical treatment followed by adjuvant chemotherapy for localized small-cell lung cancer (SCLC), the results of treatment in 86 SCLC patients since 1990 were reviewed. Of 86 patients, 19 patients received pneumonectomy, 53 lobectomy, 6 wedge resection and 8 segmental resection. Postoperative pathologic staging revealed 24 in stage I, 36 in stage II, and 26 in stage III. Postoperative adjuvant chemotherapy was given in 78 patients. Until the latest follow-up, 28 patients remained alive and 58 patients died. The 5-year survival rate of the 86 patients was 37%. The survival of patients in stage I was better than that of patients in stage II (P = 0.018) and stage III (P = 0.021), but the survival of patients in stage II was not significantly different from that of patients in stage III (P = 0.234). In summary, surgical treatment followed by chemotherapy improves significantly the survival of patients with localized SCLC. TNM staging is of prognostic importance.

Study Design: The mean 1-year cost of treating 66 Medicare Advantage patients with diabetes and CKD who were receiving PIR was compared with that of treating 1301 Medicare Advantage patients with diabetes and CKD not receiving PIR. Differences in disease severity were compared using mean risk adjustment factor scores.

Combination therapies using checkpoint inhibitors with immunostimulatory agonists have attracted great attention due to their synergistic therapeutic effects for cancer treatment. However, such combination immunotherapies require specific timing of doses to show sufficient antitumor efficacy. Sequential treatment usually requires multiple administrations of the individual drugs at specific time points, thus increasing the complexity of the drug regimen and compromising patient compliance.

This study aims to identify common clinical trajectories based on dynamic assessment of cardiorespiratory support using a validated electronic health record data that covers retrospective cohort of 19,177 patients with sepsis admitted to ICUs of Mayo Clinic Hospitals over eight-year period. Patient trajectories were modeled from ICU admission up to 14 days using an unsupervised machine learning two-stage clustering method based on cardiorespiratory support in ICU and hospital discharge status.
