Spinal subdural empyema (SE) is a rare condition. We describe a young patient with a torathic SE after lumbar epidural anaesthesia. SE was distant from the region of manipulation. Spinal magnetic resonance was the most useful procedure for diagnosis and follow-up. Treatment with intravenous antibiotics and drainage was not enough, and she needed complete surgical excision of the lesion.

Article Synopsis
  • * MRI revealed leptomeningeal enhancement and later showed a subdural lesion at the D4 and D5 vertebrae, leading to surgical intervention where an abscess and granuloma were found.
  • * Histopathological analysis confirmed mucormycosis, a fungal infection, and after over 8 weeks of treatment with antifungal medication, the patient experienced complete neurological recovery.

Commentary on "Structural characterization of SLYM - a 4th meningeal membrane".

Fluids Barriers CNS

September 2024

Department of Immunology, Genetics and Pathology, Rudbeck Laboratory, Uppsala University, Uppsala, Sweden.

For centuries, the meninges have been described as three membranes: the inner pia, middle arachnoid and outer dura. It was therefore sensational when in early 2023 Science magazine published a report of a previously unrecognized - 4th - meningeal membrane located between the pia and arachnoid. Multiple features were claimed for this new membrane: a single cell layer marked by the transcription factor Prox1 that formed a barrier to low molecular weight substances and separated the subarachnoid space (SAS) into two fluid-filled compartments, not one as previously described.

Objective: The purpose of this report is to describe a case series of children undergoing selective dorsal rhizotomy (SDR) for the treatment of spastic cerebral palsy in Vietnam. Also described is an international cooperation model to facilitate the development of a new, multidisciplinary team for the evaluation and treatment of these children.

Methods: Details of international collaboration are described, including in-person travel and virtual interactions.

Considerations for implanting speech brain computer interfaces based on functional magnetic resonance imaging.

J Neural Eng

May 2024

Department of Neurology and Neurosurgery, University Medical Center Utrecht Brain Center, Utrecht University, Utrecht, The Netherlands.

Brain-computer interfaces (BCIs) have the potential to reinstate lost communication faculties. Results from speech decoding studies indicate that a usable speech BCI based on activity in the sensorimotor cortex (SMC) can be achieved using subdurally implanted electrodes. However, the optimal characteristics for a successful speech implant are largely unknown.
