Difficult patients challenge the resourcefulness of nurses. Considering the results of a focus group study, guidelines are designed for the care of difficult patients. Four case studies present the use of the guidelines, including interventions and outcomes.

Introduction: Bipolar disorder (BD) and major depressive disorder (MDD) have overlapping clinical presentations which may make it difficult for clinicians to distinguish them potentially resulting in misdiagnosis. This study combined structural MRI and machine learning techniques to determine whether regional morphological differences could distinguish patients with BD and MDD.
Methods: A total of 123 participants, including BD (n = 31), MDD (n = 48), and healthy controls (HC, n = 44), underwent high-resolution 3D T1-weighted imaging.

Radiotherapy (RT) is widely applied in tumor therapy, but inevitable side effects, especially for skin radiation injury, are still a fatal problem and life-threatening challenge for tumor patients. The main components of topical radiation protection preparations currently available on the market are antioxidants, such as SOD, which are limited by their unstable activity and short duration of action, making it difficult to achieve the effects of radiation protection and skin radiation damage treatment. Therefore, we designed a drug-free antioxidant hydrogel patch with encapsulated bioactive epidermal growth factor (EGF) for the treatment of radiation skin injury.

Negative pressure wound therapy (NPWT) is a very effective method in the treatment of dehiscent, infected, and non-healing wounds. Difficult wound healing occurs especially in late pregnancy due to the rapid enlargement of the uterus and the constantly increasing tension of the entire abdominal wall. In cases of dehiscence of the surgical wound during pregnancy, proper subsequent treatment is needed, where it is necessary to consider the safety of the mother as well as the fetus.

Background: The mechanistic pathways that give rise to the extreme symptoms exhibited by rare disease patients are complex, heterogeneous, and difficult to discern. Understanding these mechanisms is critical for developing treatments that address the underlying causes of diseases rather than merely the presenting symptoms. Moreover, the same dysfunctional series of interrelated symptoms implicated in rare recessive diseases may also lead to milder and potentially preventable symptoms in carriers in the general population.

Background: Given the prevalence of cardiovascular disease, encountering difficult airways in this patient population is quite common. The challenge for anesthesiologists lies not only in establishing the airway but also in managing the hemodynamic instability caused by sympathetic activation during intubation. The purpose of this report is to describe the anesthetic experience of this patient with severe mitral and tricuspid regurgitation, atrial fibrillation with rapid ventricular response, and moderate pulmonary hypertension with an anticipated difficult airway.
