Background: Herpes simplex keratitis is the most common misdiagnosis in patients with Acanthamoeba keratitis, which is increasing in frequency and is associated with daily wear soft contact lenses. Both entities usually present as unilateral keratitis. The manifestations of superficial Acanthamoeba keratitis (i.e., unilaterality, dendriform appearance, positive response to antivirals, and decreased corneal sensation) increase the opportunity for misdiagnosis as herpes simplex keratitis. The authors have encountered six patients with Acanthamoeba keratitis in whom the correct diagnosis was delayed from 2 weeks to 3 months.

Methods: All six patients underwent testing with the Cochet-Bonnet esthesiometer and extensive pharmacologic treatment for herpes simplex keratitis. Corneal scrapings were taken between 2 and 6 weeks after the initial examination.

Results: In all six patients, corneal sensation was decreased significantly. Drug therapy was ineffective. Cultures were positive for Acanthamoeba. Five of six patients underwent penetrating keratoplasty.

Conclusions: Decreased corneal sensation has contributed to the misdiagnosis of Acanthamoeba as herpes simplex keratitis. Misdiagnosis results in delayed treatment and worse outcome. The authors found that significantly decreased corneal sensation is a frequent finding in early Acanthamoeba keratitis. Therefore, physicians should consider Acanthamoeba keratitis as an alternative diagnosis in patients with presumed herpes simplex keratitis with decreased corneal sensation.

A New Method to Measure Ocular Surface Sensitivity: Repeatability and Reproducibility of the Liquid Jet Esthesiometer.

Eye Contact Lens

November 2024

School of Optometry and Vision Science (K.E., F.S., B.T., J.C., B.G.), University of New South Wales, Sydney, Australia; and Department of Optics and Optometry and Vision Sciences (C.T.-E.), University of Valencia, Valencia, Spain.

Objectives: To assess the reliability of the liquid jet esthesiometer (LJA) for measurement of corneal sensitivity.

Methods: Two separate studies were conducted to assess intrasession repeatability (study 1) and intersession reproducibility (study 2) of corneal sensitivity measured using the LJA. Thirty participants (13 female and 17 male participants, age 23±6 years) participated in study 1 and another 30 (18 female and 12 male participants, 35±10 years) participated in study 2.

  • He had a specific corneal ulcer in the right eye and a pannus in the left eye, but tests ruled out infections.
  • Treatment with eye drops led to significant improvement, and follow-up revealed healing of the corneal lesions and improved vision.

Handling venomous snakes poses a significant risk of envenomation. In the case of spitting cobras, additional precautions, like wearing face shields, are necessary to prevent ophthalmic envenomation. Although, the Monocled Cobra (Naja kaouthia) is not a true spitting cobra, however some spitting events has been reported from Northeastern India and Nepal but ocular envenomation is scarce.

Purpose: Corneal pain is one of the most common eye symptoms caused by various types of epithelial injuries, including traumatic abrasion, chemical injury, ulcers, ultraviolet exposure, and infection. However, current therapeutic options for corneal pain are limited. In this study, we synthesized a novel quaternary ammonium compound, N-propylamiodarone bromide (NPA), and employed a rodent model of corneal injury to investigate whether NPA offers prolonged corneal analgesia through transient receptor potential vanilloid 1 (TRPV1) channel-mediated selective cellular entry, without hindering corneal epithelial recovery.
