Nontraumatic spinal epidural hematomas. MR features.

Acta Radiol

Department of Neuroradiology, Inselspital, University of Berne, Switzerland.

Published: January 1997

Purpose: Spinal epidural hematoma (SEH) is a rare clinical entity with a bleak outcome. The aim of our study was to establish the value of MR findings in the diagnosis of nontraumatic SEH.

Material And Methods: Seven patients with nontraumatic SEH were examined by MR at 1.5 T. Two patients were under anticoagulation therapy with heparin, and 2 others were taking salicylic acid. One patient had lupus erythematodes with a marked thrombocytopenia. One patient had a spinal arteriovenous malformation.

Results: MR imaging permitted the accurate localization of extradural intraspinal expansive lesions which exhibited the characteristic signal intensities of blood. Five patients underwent laminectomy and evacuation of the hematoma. In the acute phase, the hematomas appeared isointense when compared with the spinal cord on T1-weighted images and hyperintense on T2-weighted images. Later the hematomas were hyperintense on T1-weighted images and showed signals identical to those of cerebrospinal fluid (CSF) on T2-weighted images.

Conclusion: MR imaging established the exact diagnosis and localization of SEH in all cases. MR also can provide useful information about the age of the hematomas.

Intradural lumbar disc herniation (ILDH) is a very rare condition, with cerebrospinal fluid (CSF) leakage as a postoperative complication. The central canal of the conus medullaris was reported to communicate with the subarachnoid space through a caudal aperture; however, this aperture has never been observed in vivo. Herein, we report a case of L1/2 ILDH with postoperative spinal adhesive arachnoiditis and syringomyelia in which the communication considered to be a caudal aperture was detected.

Azygos Vein Stenosis in Frontotemporal Dementia Sagging Brain Syndrome.

January 2025

Ataxia Center, Cognitive Behavioral Neurology Unit, Laboratory for Neuroanatomy and Cerebellar Neurobiology, Department of Neurology (J.D.S., Massachusetts General Hospital and Harvard Medical School, Boston, Massachusetts.

Background And Purpose: Symptoms indistinguishable from behavioral-variant frontotemporal dementia (bvFTD) can develop in patients with spontaneous intracranial hypotension associated with severe brain sagging. An underlying spinal CSF leak can be identified in only a minority of these patients and the success rate of nondirected treatments, such as epidural blood patching and dural reduction surgery, is low. The disability associated with bvFTD sagging brain syndrome is high and, because of the importance of the venous system in the pathophysiology of CSF leaks in general, we have investigated the systemic venous circulation in those patients with recalcitrant symptoms.

Spinal Epidural Hematoma Following Combined Spinal Epidural Anesthesia: A Case Report.

J Perianesth Nurs

January 2025

Department of Anesthesiology, Zhejiang Hospital, Hangzhou, Zhejiang, China. Electronic address:

A spinal epidural hematoma (SEH) is a rare complication of combined spinal epidural anesthesia. The case of a 55-year-old man who underwent orthopedic surgery under combined spinal epidural anesthesia is presented. Flurbiprofen and horse chestnut seed extract that potentially affect coagulation function during the perioperative period were used.

Advances in Anesthesia Techniques for Postoperative Pain Management in Minimally Invasive Cardiac Surgery: An Expert Opinion.

J Cardiothorac Vasc Anesth

January 2025

Department of Anesthesiology, Beijing Anzhen Hospital, Capital Medical University, Beijing, China. Electronic address:

Minimally invasive cardiac surgery (MICS) often leads to severe postoperative pain. At present, multimodal analgesia schemes for MICS have attracted much attention, and the application of various chest wall analgesia techniques is becoming increasingly widespread. However, research on anesthesia techniques for postoperative pain management in MICS remains relatively limited at present.

Aim: Minimally-invasive spinal surgery is increasingly being adopted worldwide. In this study, we evaluated the postoperative magnetic resonance imaging (MRI) findings and clinical outcomes of patients who underwent full endoscopic lumbar disk surgery.

Methods: Preoperative and postoperative 3rd and 6th month MRI features, visual analog scale (VAS) score, Oswestry Disability Index (ODI), and clinical features of patients who underwent percutaneous endoscopic lumbar discectomy between August 2009 and January 2012 were retrospectively analyzed.
