We report a case of malignant paraganglioma with intracranial metastasis. A 26-year-old man presented with leg pain. On admission a metastatic bone tumor was found. General examination revealed metastatic lesions in the lung, liver, orbits, ethmoid sinus, anterior mediastinum, and brain. An MRI of the head showed multiple lesions in the skull bone and intracranial space. A biopsy was performed on the right temporal tumor, resulting in a diagnosis of malignant paraganglioma. The tumor origin could not be determined. The tumor resection was not indicated because of multiple metastasis. Both chemotherapy and radiotherapy were given to the patient, but a satisfactory outcome was not obtained.

Paragangliomas are slow-growing, extra-adrenal neuroendocrine tumors with rare intracranial presentation. Although benign, they can be locally aggressive tumors causing bone destruction and compression related symptoms. 68 Ga-DOTATATE PET/CT demonstrates superior sensitivity compared with other cross-sectional and functional imaging techniques in detecting head and neck paragangliomas. It is the primary recommended imaging modality for the evaluation of head and neck paragangliomas, especially in patients suspected of having locally aggressive, multifocal, or metastatic disease, as well as those being considered for targeted peptide-receptor therapy. Herein, we demonstrate the 68 Ga-DOTATATE PET/CT findings of a skull base paraganglioma with extensive local invasion and intracranial extension.

After two decades from its introduction in the lateral skull base paraganglioma surgery, the indications and results of preoperative internal carotid artery stenting should be critically assessed. Monocentric retrospective study on 26 patients affected by head and neck paragangliomas (19 tympanojugular paragangliomas, 4 carotid body paragangliomas, 3 vagal paragangliomas) preoperatively treated with internal carotid artery stents between 2008 and 2023. The preoperative findings, the intraoperative complications and the final surgical results were analyzed.
