Background: Balloon-assisted bioprosthetic or native aortic scallop intentional laceration to prevent iatrogenic coronary artery obstruction (BA-BASILICA) enables valve-in-valve transcatheter aortic valve implantation (ViV-TAVI) in patients at risk of coronary artery obstruction. However, its efficacy in patients with severely calcified leaflets remains unclear.

Case Summary: We report a 78-year-old woman with a deteriorated 21 mm Carpentier-Edwards PERIMOUNT Magna valve.

March 2024

Department of Thoracic Surgery, Fukujuji Hospital, Kiyose-shi Tokyo, Matsuyama, 204-8522 3-1-24, Japan.

Article Synopsis
  • A case of cardiac tamponade caused by coronary artery injury is presented, highlighting its rarity as a complication following lobectomy for lung cancer.
  • An 82-year-old man underwent left upper lobectomy, experienced a postoperative shock due to cardiac tamponade, and was found to have a lacerated coronary artery; he ultimately died from complications.
  • The study suggests that while intraoperative injury to the heart is a possibility, the injury may have been caused by staples used during surgery, which could have penetrated the pericardium as the lung expanded post-surgery.
