Background: The relationship between systemic lupus erythematosus (SLE) patients treated with rituximab and nocardiosis remains unclear.
Cases Report: A 55-year-old female with lupus nephritis II and smoking history, was treated with high-dose steroids, immunoglobulins, rituximab, and azathioprine. She developed infectious loci in the lungs, hip, and brain.

A 56-year-old woman with a background of neuromyelitis optica spectrum disorder associated with aquaporin-4 antibodies (AQ4-NMOSD) treated with azathioprine, presented with a 2-month history of declining mobility, weight loss and hoarse voice. She had a history of autoimmune thyrotoxicosis and treated hypertension. Given her smoking history, the initial clinical concern was of malignancy.

The association between psoriasis and non-melanoma skin cancer (NMSC) remains inconsistent despite biologic plausibility. Immunosuppressive effects of systemic psoriasis treatments have also been hypothesized to contribute to the development of NMSC as well. However, data assessing the risk of NMSC associated with immunomodulatory psoriasis medications, particularly newer biologic therapies, are limited.
