Background: Atrial standstill is characterized by the absence of atrial activity. We report a case of a patient with extensive atrial fibrosis who underwent electrophysiologic study (EPS) and electroanatomical mapping (EAM) to identify surviving atrial sites amenable for pacemaker lead implantation.
Case Summary: A 72-year-old man with persistent atrial fibrillation (AF) and atrial functional mitral regurgitation/tricuspid regurgitation (MR/TR) underwent a Cox-Maze surgery, mitral and tricuspid valve repair, and biatrial plication.

We report a case of atrial functional tricuspid regurgitation with an atrial standstill in a 71-year-old woman with a history of chronic atrial fibrillation (AF). The ECG showed a flat baseline with no AF waves and regular, narrow QRS complexes, whereas the previous ECG demonstrated AF. Echocardiography revealed dilation of the right atrium and the tricuspid annulus with severe regurgitation, but mitral regurgitation was mild.

Clinicians should recognize ventricular standstill as a complication of cardiac revascularization and be cognizant of asymptomatic cases necessitating intervention. Early evaluation to facilitate pacemaker implantation portends good outcomes in this patient subgroup.

It has a rapid onset of action and an intense and consistent platelet reactivity inhibition that has been demonstrated to be superior to clopidogrel in decreasing major adverse events in acute coronary syndrome (ACS). Although ticagrelor is well tolerated in ACS patients, it has side effects, such as dyspnea and bradyarrhythmia, as reported in the Platelet Inhibition and Patient Outcomes (PLATO) study.
