Optimizing TMS dosimetry: evaluating the effective electric field as a novel metric.

Objective: This study introduces the effective electric field (Eeff) as a novel observable for transcranial magnetic stimulation (TMS) numerical dosimetry. Eeff represents the electric field component aligned with the local orientation of cortical and white matter neuronal elements. To assess the utility of Eeff as a predictive measure for TMS outcomes, we evaluated its correlation with TMS induced muscle responses and compared it against conventional observables, including the electric (E-)field magnitude, and its components normal and tangential to the cortical surface.

Introduction: Cerebral small vessel disease (CSVD) is a chronic systemic degenerative disease affecting small blood vessels in the brain, leading to cognitive impairments. Transcranial direct current stimulation (tDCS), a non-invasive brain stimulation technique that applies low electrical currents to the scalp, shows promise in treating cognitive and movement disorders. However, further clinical evaluation is required to assess the long-term effects of tDCS on neuroplasticity and gait in patients with CSVD.

Acute internal carotid artery occlusion (AICAO) can result in malignant cerebral edema and unfavorable patient outcomes. This study evaluated the utility of transcranial Doppler (TCD) in assessing contralateral flow compensation and predicting outcomes in patients with AICAO. Background And Objectives: Recently, reduction of transcallosal inhibition by contralateral navigated repetitive transcranial magnetic stimulation (nrTMS) improved neurorehabilitation of glioma patients with new postoperative paresis. This multicentric study examines the effect of postoperative nrTMS in brain tumor patients to treat surgery-related upper extremity paresis.

Methods: This is a secondary analysis of two randomized and three one-arm studies in brain tumor patients with new/progressive postoperative paresis.
