The case of a 78-year-old man with retroperitoneal hemorrhage due to a ruptured right inferior suprarenal artery aneurysm is presented. The diagnosis was made by angiography which allowed immediate embolization of the neck of the aneurysm, controlling hemorrhage and obviating the need for surgery. The patient made a full recovery with no evidence of further hemorrhage.

Transaortic endarterectomy (TE) is an effective and durable method of restoring patency in the aorta afflicted with atherosclerotic disease, which most commonly affects the infrarenal aorta and common iliac artery. When the suprarenal aorta is involved, the disease is usually confined to the orifices of the visceral vessels without obstruction of the aortic lumen. In rare cases, dense, calcified, exophytic, and amorphous lesions causing severe luminal obstruction, termed coral reef atherosclerosis (CRA) of the suprarenal aorta, may occur.

We demonstrate an aberrant ramification pattern of the renal and testicular vessels. On both sides, the anterior and posterior renal veins emerged from the renal. On the right side, the anterior renal vein collected the right testicular vein and drained into the inferior vena cava, while the posterior one directly drained into the inferior vena cava.
