Laparoscopic approach for treatment of a primary splenic cyst.

We present the case of a 24-year-old woman with a symptomatic primary splenic cyst, and we relate our first experience with laparoscopic resection of the extrasplenic wall of the cyst. The procedure was successful, and the patient was discharged asymptomatic two days following surgery. We describe the surgical technique used and comment on some pathologic and tactical aspects. We conclude by remarking on the association of the beneficial advantages of minimally invasive surgery with the demand for more conservative splenic surgical procedures. Therefore, laparoscopic partial cystectomy may be a useful alternative for patients who require an effective treatment for nonparasitic splenic cysts.

Background: Pleural effusion, the presence of fluid within the pleural space, is a common condition secondary to a wide range of pathological causes. Splenic abscess, which is rare, has previously been described as a cause of exudative pleural effusion. Splenic abscess is thought to be associated with bacteremia, iatrogenic inoculation, or hematogenous spread from another bacterial focus.

Background: Non-erosive reflux disease (NERD), the most frequent phenotype of gastroesophageal reflux disease, presents without visible esophageal mucosal damage but significantly impacts patients' quality of life. Current treatments like proton pump inhibitors show limited efficacy for many NERD patients, necessitating alternative approaches. Jianpi Qinghua (JQ) granules, a traditional Chinese medicine, have shown promise in treating NERD by targeting symptoms of spleen deficiency and damp-heat syndrome.

Splenomegaly is a quite common clinical feature of Philadelphia (Ph) negative chronic myeloproliferative neoplasms (MPNs) and its presence may, in some cases, drives treatment decision. Most importantly, palpable splenomegaly is a minor criterion for both pre-fibrotic/early primary myelofibrosis and primary myelofibrosis (PMF) diagnosis, even if clinical assessment by physical examination is poorly reliable and accurate. On the other hand, despite the International Working Group-Myeloproliferative Neoplasms Research and Treatment and European LeukemiaNet guidelines defined spleen response criteria by palpation, they also recognized the highly subjective nature of spleen size assessment by physical examination, and recommended objective confirmation of volume reduction via computed tomography or magnetic resonance imaging (MRI).

Background: In China, 80% of Hepatocellular Carcinoma (HCC) is associated with cirrhosis. Portal hypertension, the most common outcome of cirrhosis progression, has a high incidence. Platelet count/spleen diameter ratio (PSL) with a cut-off value of 909 can predict the presence of esophagogastric varices and thus portal hypertension, which is also an independent risk factor for early recurrence and late recurrence of hepatocellular carcinoma after resection.
