[Severe bradyarrythmia induced by tricyclic antidepressants in an elderly patient].

Rev Assoc Med Bras (1992)

Departamento de Medicina, Santa Casa de São Paulo.

Published: December 1996

A 66 year-old female patient, with severe chronic pain caused by diabetic radiculopathy, treated with association of amitriptiline and carbamazepine, presented severe sinusal bradycardia with hemodynamic disturbances ensuing junctional rhythm, which required installation of temporary pace-maker. The drugs were withdrawn. Reversion to sinusal rhythm occurred 36 hours after the installation of bradycardia. The pace-maker was withdrawn. It was concluded that the use of tricyclic antidepressive drugs, particularly in the elderly, should be carefully monitored.

Etrasimod is a sphingosine 1 phosphate (S1P) receptor modulator approved for the treatment of moderate to severely active ulcerative colitis (UC). Etrasimod selectively activates S1P receptors with no detectable activity on S1P. The ELEVATE clinical trials evaluated the efficacy and safety of etrasimod for UC.

Importance: Voltage-gated potassium channel complex antibody (VGKCc-Ab) encephalitis is an immunotherapy-responsive syndrome usually associated with causative antibodies that target the leucine-rich, glioma inactivated 1 (LGI1) protein. Although it is expressed throughout the brain, LGI1 is not known to be expressed in cardiac tissue. We describe a novel neurocardiac prodrome of VGKCc-Ab/LGI1-encephalitis.

Cardiac changes may occasionally occur during vagus nerve stimulation (VNS) used in epileptic patients. As they can be potentially life-threatening, it is important to detect them, and this is why an intraoperative test is performed during the implantation. Few cases of asystole during this test have been described.

Patients who have silent sick sinus syndrome (SSS) can show various unexpected arrhythmias during surgery. The severity of these bradyarrythmias is affected by anesthetic methods. We report a unique case of a patient with silent SSS who developed 40 s of asystole under combined general and epidural anesthesia.
