To determine the choice of flap cover for patients presenting with bilateral lower limb trauma requiring free flap cover and to derive a step-wise guide to the planning of bilateral lower limb free flaps. This was a retrospective study of patients over a 20 year period from 2000 to 2020 who presented with bilateral lower limb defects following trauma and were managed with two free flaps for wound cover, done either simultaneously or sequentially in the same admission. Of the 11 cases with 22 defects, there were 3 re-explorations with 1 flap loss managed with delayed fasciocutaneous flap cover.

The growing disparity between the demand for pancreas transplants and the availability of suitable organs underscores the urgent need for innovative donor strategies, including the utilization of donors after circulatory death (DCD). This scoping review presents a comprehensive comparative analysis of transplantation outcomes between DCD and donors after brain death (DBD), focusing on pancreatic graft survival, postoperative complications, and functional metrics such as graft performance and HbA1c levels. Although DCD grafts were suspected to be associated with higher rates of early complications, including delayed graft function and thrombosis, altogether resulting from potentially more ischemia-reperfusion injuries, their long-term outcomes are comparable to those of DBD grafts.

Background: The functional reconstruction of bone defects following resection of proximal humerus tumors in children poses a significant challenge. This study utilized vascularized fibular epiphyseal transfer for proximal humerus reconstruction to evaluate the outcome, complications, and survival rates.
Methods: In this study, we conducted a retrospective analysis of 13 pediatric patients who underwent vascularized fibular epiphyseal transfer for biological reconstruction following oncologic resection of the proximal humerus between 2019 and 2021.

A quinquagenarian underwent zone 2 arch repair for acute type A dissection followed by endovascular repair utilizing a branch endoprosthesis and covered stents. He developed a fever and positive blood culture results 3 weeks after the thoracic endovascular repair. A preoperative left carotid to subclavian artery bypass was performed.

The vast majority of patients with advanced chronic kidney disease (CKD) who transition to end-stage kidney disease (ESKD) are treated with dialysis. Given that dialysis does not always have the intended effects of increasing longevity and/or improving health, particularly in those with high comorbidity burden and/or older age groups, there has been increasing emphasis on interventions that delay or avert the need for renal replacement therapy. Among the multi-disciplinary approaches used to reduce CKD progression, dietary interventions are a major cornerstone.
