To prospectively evaluate the feasibility and efficacy of neoadjuvant chemoradiotherapy locally advanced or recurrent vulvar carcinoma, 58 patients referring for primary (41) or recurrent (17) disease received preoperative external radiotherapy to a dose of 54 Gy, divided into two courses with an interval of 2 weeks. 5-Fluorouracil (750 mg/m2 daily for 5 days) and mitomycin-C (15 mg/m2 single bolus) were given at the start of each cycle. Wide local excision and inguinal lymphadenectomy were planned after treatment. Eighty-nine percent of patients completed the chemoradiotherapeutic treatment, whereas 72% underwent surgery. Objective responses were observed in 80% of vulvar diseases and in 79% of groin metastases. Pathologic complete response of both the vulvar and inguinal disease was confirmed in 13 patients (31%). Early severe toxicity was recorded in 3 patients and severe worsening of performance status in 3. Three deaths occurred shortly after treatment and at least one is directly related to toxic effects. This treatment allows good control of locally advanced and recurrent vulvar cancer with acceptable side effects. Further follow-up is required to determine the long-term outcome and the effectiveness of the surgical procedure.

Treatment approaches in advanced penile cancer: targeted therapies and immunotherapy.

Penile cancer is a rare genitourinary malignancy which can be treated with surgery or radiation for localized disease, but often requires systemic treatment with chemotherapy for recurrent or metastatic disease. With the emergence of immune checkpoint inhibitors and targeted therapies for specific genomic aberrations in the treatment of over a dozen other cancers, recent studies have sought to identify therapies other than chemotherapy in treating this uncommon cancer. Several ongoing trials involving immune checkpoint inhibitors, tyrosine kinase inhibitors, and antibody drug conjugates are attempting to identify additional therapies.

A reward shaping deep deterministic policy gradient (RS-DDPG) and simultaneous localization and mapping (SLAM) path tracking algorithm is proposed to address the issues of low accuracy and poor robustness of target path tracking for robotic control during maneuver. RS-DDPG algorithm is based on deep reinforcement learning (DRL) and designs a reward function to optimize the parameters of DDPG to achieve the required tracking accuracy and stability. A visual SLAM algorithm based on semantic segmentation and geometric information is proposed to address the issues of poor robustness and susceptibility to interference from dynamic objects in dynamic scenes for SLAM based on visual sensors.

Background: Adding pembrolizumab, an anti-PD-1 antibody approved for treatment of head and neck squamous cell carcinoma (HNSCC) to neoadjuvant (induction-) chemotherapy utilizing docetaxel and cisplatin (TP) followed by radiotherapy may improve outcome in larynx organ-preservation (LOP) that is investigated in the European Larynx-Organ preservation Study (ELOS). As biomarkers for response to TP and pembrolizumab +TP are missing but may include cytokines, this work aims on determining cytokines potentially linked to outcome as prognostic markers sufficient to predict and/or monitor response to successful LOP.

Methods: Collagenase IV digests were generated from 47 histopathological confirmed HNSCC tumor samples and seeded in 96-well plates containing pembrolizumab, docetaxel, cisplatin either solely or in binary or ternary combination.
