AI Article Synopsis

  • A study analyzed 248 patients with advanced squamous cell carcinoma of the pyriform sinus from 1980 to 1985, focusing on those who underwent total pharyngolaryngectomy followed by radiotherapy.
  • The study found a 27.4% local regional recurrence rate, with factors like clinical staging and residual tumor affecting failure rates.
  • Overall, the 5-year survival rate was 33%, with a median survival of 32 months, highlighting the poor prognosis despite aggressive treatment approaches.

Article Abstract

Between January 1980 to December 1985, 248 patients with advanced squamous cell carcinoma of the pyriform sinus were retrospectively analysed. Criteria for inclusion in the study were the following: no previous treatment and treatment combining total pharyngolaryngectomy and postoperative radiotherapy. Mean follow up was 5 years with a minimum of 3 years. Seventy-one patients had a local regional recurrence (27.4%). Clinical staging at presentation and residual tumor at the primary site after surgery were factors significantly associated with higher local failure rates. The 5 year survival rate was 33% and the median survival time was 32 months (plateau was reached after the 6th year). The most frequent severe complication observed was pharyngeal stenosis occurring in 6% of the cases. This study confirms the poor prognosis of squamous cell carcinoma of the pyriform sinus, in spite of the combination of radical surgery and high dose postoperative radiotherapy.

Application of pedicled supraclavicular flaps in hypopharyngectomy with preservation of laryngeal function.

BMC Surg

December 2024

Department of Otolaryngology Head and Neck Surgery, The Affiliated Hospital of Southwest Medical University, No.25, Taiping Street, Jiangyang District, Luzhou, 646000, China.

Objective: To evaluate the efficacy of pedicled supraclavicular flaps in hypopharyngectomy reconstruction, with a focus on preserving laryngeal function.

Methods: From August 2019 to June 2022, 14 patients with primary hypopharyngeal carcinoma who met the inclusion and exclusion criteria and underwent the repair of hypopharyngeal defects using pedicled supraclavicular flaps were included retrospectively. Relevant clinical evaluation indicators include patient characteristics, defect sizes, flap sizes, flap harvesting time, postoperative hospital stay, postoperative complications, recurrence, and survival outcomes.

Introduction: Hypopharyngeal squamous cell carcinoma, stage III has poor prognosis with only 25% chance of 5 years of relative survival in such patients in spite of conventional treatment including radical surgery, radiotherapy, and concurrent chemotherapy.

Case Presentation: A chronic tobacco-betel nut chewer 62-year-old male patient had dysphagia with hoarseness of voice diagnosed with stage III, grade II malignant pyriform fossa. The patient underwent 9 cycles of neoadjuvant chemotherapy with Inj Paclitaxel 100 mg and Inj Cisplatin 40 mg.

Article Synopsis
  • The study explored the role of minor salivary glands in the hypopharynx as potential markers for ensuring safe surgical removal of superficial hypopharyngeal cancers while minimizing complications.
  • The research involved examining 23 cadaver specimens to assess the presence, size, and depth of minor salivary glands across different hypopharyngeal regions and analyzing resected tissues from 5 patients with cancer.
  • Findings indicated that these glands were commonly found, especially in the postcricoid area, and their locations could help determine safe margins during surgery, suggesting they are effective indicators for surgical strategies.

To investigate the pathological characteristics of tumor regression and the expression level of chemoradiotherapy resistance-related molecular markers after preoperative concurrent radiochemotherapy in patients with locally advanced hypopharyngeal carcinoma. The clinical data of 44 patients with locally advanced hypopharyngeal carcinoma who underwent preoperative concurrent radiochemotherapy in the Department of Head and Neck Surgery of Shandong Otolaryngology Hospital from August 2016 to August 2020 were retrospectively analyzed. All patients received preoperative concurrent chemotherapy and radiotherapy.
