In March 1997 a distinguished historian, Professor Joel Williamson of the University of North Carolina, Chapel Hill, wrote a self-reflective essay in the () in which he examined why he, as a born and bred Southerner, could not see and in fact denied and obviated the existence of lynching in American history. The , with an exemplary psychodynamic introduction by editor David Thelen, persuaded six referees, all distinguished scholars, four white and two African American, to waive their strict confidentiality and publish their reviews. The author published the paper as submitted, with no revisions.

Objectives: To explore the provision of medical care at 'unlicensed', full-contact amateur and lower-level professional combat sports competitions in England.
Design: Qualitative, mixed methods.
Methods: Observations totalling 200h of fieldwork shadowing medical professionals at 27 individual combat sports events, alongside formal, semi-structured interviews with 25 medical professionals, 7 referees and 9 promoters/event staff.

A publication is an important narrative of the work done and interpretations made by researchers securing a scientific discovery. As The Royal Society neatly states though, "Nullius in verba" ("Take nobody's word for it"), whereby the role of the underpinning data is paramount. Therefore, the objectivity that preserving that data within the article provides is due to readers being able to check the calculation decisions of the authors.

We present a taxonomy of errors in the scientific literature and an account of how the errors are distributed over the categories. We have developed the taxonomy by studying substantial errors in the scientific literature as described in retraction notices published in the journal over the past 35 years. We then examine how the sorts of errors that lead to retracted papers can be prevented and detected, considering the perspective of collaborating scientists, journal editors and referees, and readers of the published articles.
