Effect of melanin, oxyhemoglobin and bilirubin on transcutaneous bilirubinometry.

Published: January 1993

To determine the effect of skin pigments on transcutaneous bilirubinometer readings, we measured the effect of bilirubin, melanin, and oxyhemoglobin solutions on transcutaneous bilirubinometer readings in vitro. Our results showed that the variability of the readings in vitro was related to the instrument's non-linear response to bilirubin and melanin concentration and an inaccurate oxyhemoglobin correction factor. These factors should be considered in developing a more accurate non-invasive method of monitoring serum bilirubin concentration.

: Based on Blind Source Separation and the use of multispectral imaging, the new approach we propose in this paper aims to improve the estimation of the concentrations of the main skin chromophores (melanin, oxyhemoglobin and deoxyhemoglobin), while considering shading as a fully-fledged source. : In this paper, we demonstrate that the use of the Infra-Red spectral band, in addition to the traditional RGB spectral bands of dermatological images, allows us to model the image provided by each spectral band as a mixture of the concentrations of the three chromophores in addition to that of the shading, which are estimated through four steps using Blind Source Separation. : We studied the performance of our new method on a database of real multispectral dermatological images of melanoma by proposing a new quantitative performances measurement criterion based on mutual information.

Oximetry is used to quantify the presence of oxygen in soft tissues. It can be expressed as, for example, tissue oxygen saturation (StO), arterial oxygen saturation (SaO) and pulsatile oxygen saturation (SpO), among others. Non-invasive medical devices are used to estimate (SaO).

Significance: Near-infrared spectroscopy (NIRS) is a non-invasive optical method that measures changes in hemoglobin concentration and oxygenation. The measured light intensity is susceptible to reduced signal quality due to the presence of melanin.

Aim: We quantify the influence of melanin concentration on NIRS measurements taken with a frequency-domain near-infrared spectroscopy system using 690 and 830 nm.

Significance: Hyperspectral cameras capture spectral information at each pixel in an image. Acquired spectra can be analyzed to estimate quantities of absorbing and scattering components, but the use of traditional fitting algorithms over megapixel images can be computationally intensive. Deep learning algorithms can be trained to rapidly analyze spectral data and can potentially process hyperspectral camera data in real time.

Background: Pulse oximeters work within the red-infrared wavelengths. Therefore, these oximeters produce erratic results in dark-skinned subjects and in subjects with cold extremities. Pulse oximetry is routinely performed in patients with fever; however, an elevation in body temperature decreases the affinity of hemoglobin for oxygen, causing a drop in oxygen saturation or oxyhemoglobin concentrations.
