A modification of the Sanvenero-Rosselli velopharyngoplasty.

Published: January 1993

A modification of the cranially-pedicled velopharyngoplasty is described. It consists of the preparation of two mucosal flaps on the dorsal side of the soft palate. They are pedicled on the velar rim and sutured to the free part of the pharyngeal flap between its origin and the recipient bed. Advantages of the method comprise reliable healing, the breadth of the flap and an improved velopharyngeal occlusion with results which favourably influence speech.

Article Synopsis
  • The study focuses on improving speech in patients with velopharyngeal incompetence through surgical techniques, particularly comparing Sanvenero-Rosselli and Hogan's modification of velopharyngoplasty.
  • Over 20 years, 17 patients underwent a secondary posterior pharyngeal flap surgery, with nine treated by the Sanvenero-Rosselli method and eight by Hogan's technique, which involves creating two mucosal flaps on the soft palate.
  • Results showed significant improvements in speech for both methods, but Hogan's modification yielded better outcomes, suggesting its advantages warrant the extra surgical steps involved.

The velopharyngoplasty performed using the popular Sanvenero-Rosselli method improves the speech quality of patients with cleft palate suffering from persistent velopharyngeal insufficiency despite successful closure of the hard and soft palates. However, often a relatively narrow pharyngeal bridge results due to healing not only by granulation and scar contraction, but also due to the insertion of the inferior tip of the flap into a narrow bed. Elevation of two velar flaps with nasal mucosa to cover the exposed muscular undersurface of the pharyngeal flap produces a broader recipient bed into which the pharyngeal flap can be spread.

The present study deals with various factors affecting speech, particularly its resonance, in patients with isolated cleft palate. For that purpose a method to evaluate hypernasality was developed. The degree of hypernasality was assessed in terms of hypernasality indexes by means of a modified cul-de-sac hypernasality test.
