A nonfatal case of accidental prolonged overdosage of pyrimethamine (10 times the usual dose for 10 days) in a seven-week-old infant treated for congenital toxoplasmosis is presented. The drug was identified and quantitated in plasma and urine using an HPLC technique with diode-array detection. Initial plasma concentration of pyrimethamine was 6.22 micrograms/mL. Results are discussed in light of the existing literature.

Background: Dexmedetomidine (DMED) is frequently used as a sedative in several medical fields. The benefits of DMED include enhanced quality of regional anesthesia, prolonged analgesia, and postoperative opioid-sparing when administered intravenously or perineurally in combination with regional anesthesia. Severe hemodynamic complications, such as profound bradycardia and hypotension, can occur after DMED administration in critically ill patients or overdosage; however, there are few reports of complications with DMED administration following brachial plexus block (BPB).

Paracetamol (acetaminophen) was marketed in the 1950s as a nonprescription analgesic/antipyretic without any preclinical toxicity studies. It became used increasingly for self-poisoning, particularly in the UK and was belatedly found to cause acute liver damage, which could be fatal. Management of poisoned patients was difficult as maximum abnormalities of liver function were delayed for 3 days or more after an overdose.
