A patient with a large atrial septal aneurysm mimicking a right atrial mass is reported. Diagnosis was correctly obtained by transesophageal echocardiography. Clinical importance of this entity and usefulness of transesophageal echocardiography in its diagnosis is discussed.

Background: Plasma NT-proBNP (N-terminal pro-B-type natriuretic peptide) is commonly used to diagnose heart failure with preserved ejection fraction (HFpEF), but its diagnostic performance in the ambulatory/outpatient setting is unknown because previous studies lacked objective reference standards.
Methods: Among patients with chronic dyspnea, diagnosis of HFpEF or noncardiac dyspnea was determined conclusively by exercise catheterization in a derivation cohort (n=414), multicenter validation cohort 1 (n=560), validation cohort 2 (n=207), and a nonobese Japanese validation cohort 3 (n=77). Optimal NT-proBNP cut points for HFpEF rule out (optimizing sensitivity) and rule in (optimizing specificity) were derived and tested, stratified by obesity and atrial fibrillation.

Purpose: We hypothesized that male and female volunteers would exhibit distinct changes in cardiac morphology, systolic, and diastolic function following endurance (END) and resistance (RES) training.
Methods: Thirty-eight females and 26 males participated in a randomized cross-over design trial in which all participants completed 12-weeks END and RES, separated by a 12-week washout. Echocardiograms assessed morphology (left ventricular mass, LVM), systolic function (ejection fraction, EF, and global longitudinal strain, GLS), diastolic function (mitral valve velocities, E, A; tissue Doppler velocities, e', a'), and left atrial volume.

This case presents a curious diagnosis in a young male presenting with chest pain. The first imaging tests suggested the presence of a hypovascular left atrial tumor. After cardiac magnetic resonance and the exclusion of extra-cardiac lesions, sarcoma emerged as the main diagnostic hypothesis.
