Developing nurse education in Kenya.

This article describes a six-month project to facilitate educational developments in the School of Nursing at the Nairobi Hospital, Kenya. The project addressed curriculum revision, mentorship, audit of clinical learning environment, developing teaching and assessing skills in qualified staff, establishing a computerised student allocation system, and post-basic and in-service education. The first three will be described in detail, with some reflections on the experience of introducing change in an environment which was both familiar and strange to the author.

Background: Effective detection of cognitive impairment in the primary care setting is limited by lack of time and specialized expertise to conduct detailed objective cognitive testing and few well-validated cognitive screening instruments that can be administered and evaluated quickly without expert supervision. We therefore developed a model cognitive screening program to provide relatively brief, objective assessment of a geriatric patient's memory and other cognitive abilities in cases where the primary care physician suspects but is unsure of the presence of a deficit.

Methods: Referred patients were tested during a 40-min session by a psychometrist or trained nurse in the clinic on a brief battery of neuropsychological tests that assessed multiple cognitive domains.

Background: The rising cost of healthcare is a concerning issue for healthcare systems. The Diagnosis Related Group (DRG) system lacks direct consideration for costs related to nursing care. Therefore, to date there is no clear picture of billing models that consider also nursing activity when evaluating healthcare service costs or what factors related to nursing care affect the costs of healthcare services and would therefore need to be considered in billing models.
