Background: Arteriovenous shunts below conus medullaris (AVS-BC) are understudied, particularly those associated with spinal dysraphism. This study aimed to refine the classification and management of AVS-BC.

Methods: A retrospective analysis of patients with AVS-BC from two centers over two decades was performed, focusing on clinical presentations, angioarchitecture, and treatment outcomes.

Article Synopsis
  • Spinal dural arteriovenous fistulae (sDAVF) are common vascular malformations in the spine, necessitating surgical intervention to prevent neurological issues, but there’s limited research comparing surgical methods.
  • A systematic review analyzed various surgical techniques used to treat sDAVF and found variability in approaches, with single-level laminectomy being the most common.
  • The study identified 16 significant open questions regarding surgical methods and the need for further research to improve treatment standards and techniques.

A spontaneous spinal subdural hematoma (SSSH) is a rare but potentially deadly condition characterized by the presence of blood in the subdural space, commonly causing compression of the spinal cord and acute neurological deficits. Urgent surgical intervention with a decompressive laminectomy is warranted to avoid lasting deficits. The literature on this pathology is scarce, and the etiology is still poorly understood, although associations have been established with arteriovenous malformations, rupture of epidural vessels, and anticoagulant use, among others.

Article Synopsis
  • Epidural arteriovenous fistulas (eAVFs) are rare vascular issues often confused with intradural ones, impacting treatment and outcomes.
  • A case of a 68-year-old man initially diagnosed with a likely intradural fistula was actually found to have an epidural lesion during surgery, leading to significant improvement after treatment.
  • The findings emphasize the need for better imaging techniques and a collaborative medical approach to accurately identify and treat spinal vascular malformations.

Article Synopsis
  • * A 60+ year-old male with HHT type 1 experienced severe back pain and recurrent infections, including a serious case of MSSA bacteraemia and spinal issues, despite previous treatments for infections.
  • * The patient was successfully treated with intravenous nafcillin and prescribed cefalexin to prevent future infections, highlighting the need for clearer guidelines on preventive treatment for high-risk HHT patients.
