Results have been analysed of comprehensive evaluation of 175 patients with discirculatory encephalopathy of atherosclerotic genesis, consisting of inspection of the neck vessels projection, study of neurological status, ultrasonic investigation of the major brain arteries structure and hemodynamics, their angiographic features. At stage I discirculatory encephalopathy subjective complaints predominated, being caused by syndrome of thoracic outlet, steal syndrome, stenosing lesions of brachiocephalic arteries. Encountered more frequently in the middle-aged and elderly patients were affections of several major brain arteries. Specific weight of correlation between ultrasonic investigation and angiography in stenosing lesions of brachiocephalic arteries in patients with discirculatory encephalopathy constituted 84-100%. Therefore patients with discirculatory encephalopathy of atherosclerotic genesis require ultrasonic investigation of brachiocephalic arteries with the purpose of finding out exactly the pathological process and choosing the tactics of treatment.

In the International Classification of Diseases 11th revision in the section «Diseases of the nervous system», it is proposed to distinguish «Cerebrovascular disorder with neurocognitive impairment», which corresponds to both discirculatory encephalopathy (DEP) or chronic cerebral ischemia (CCI) accepted in our country, and also vascular cognitive impairments. The terminology, prevalence, risk factors and pathological basis of the disease are discussed, in particular multiple infarctions, strategic infarctions, cerebral small vessel disease, specific microangiopathies, intracerebral hemorrhage and global hypoperfusion. Post-stroke cognitive impairments are discussed in detail.

The effectiveness of neurometabolic therapy in patients with various stages of discirculatory encephalopathy was assessed according to the severity of cognitive impairment. Using a number of studies [clock drawing test, MMSE, Spielberger-Hanin, Bourdon test and cognitive evoked potentials (P300 peak)], cognitive functions of 60 elderly patients (mean age - 61,3±12,4 years) with various degrees of chronic head ischemia were analyzed brain and discirculatory encephalopathy of the I-III stages. Depending on the treatment regimen, patients were divided into 2 groups: 34 (the main group) received Cytoflavin in the treatment regimen: 2 tablets 2 times/day for 2 months.
