Barium ingestion is a routine practice in the radiologic examination of the alimentary tract. Barium intravasation is a rare complication, often resulting in the rapid onset of hypotension and death. The literature on this type of complication is sparse. In the majority of reported cases, the event involved high pressure barium instillation. This case illustrates the rare occurrence of barium in the portal venous circulation after an upper gastrointestinal examination.

Barium is used with great frequency for various gastrointestinal radiographic studies. Complications arising from the use of barium are uncommon and can range from peritonitis, pneumonitis, vascular intravasation, allergic reactions, and even "barium appendicitis." We report a case of an unusual complication, periostitis, from the use of barium in a 46-year-old male.

We report a case of venous intravasation of barium sulphate occurring during a routine barium enema examination for investigation of rectal bleeding. The patient suffered a cardiopulmonary arrest, but made a full recovery after organ support in intensive care. Review of radiographs from the examination showed intravasated barium in pelvic vessels.
