Assisted circulation techniques are an outgrowth of extracorporeal pump oxygenator systems developed in the mid 1950s. But the differences in physiology of total perfusions and of parital perfusions for assisting circulation delayed the clinical application of the intra-aortic balloon. The problems and risks in providing circulation assistance and the approaches used to overcome them are discussed. The various counterpulsation techniques are described and data presented to demonstrate the increased survival in patients with myocardial infarction and cardiogenic shock.

In this study, we aimed to assess the effects of enhanced external counterpulsation (EECP) and individual shear rate therapy (ISRT) on peripheral artery function in patients with lower extremity atherosclerotic disease (LEAD). We randomly assigned 45 LEAD patients to receive 35 sessions of 45 min of EECP (n = 15), ISRT (n = 15), or sham-control (n = 15). Flow-mediated dilation in the brachial artery (brachial-FMD); 6-min walk distance; blood flow in the popliteal, posterior tibial, anterior tibial, and dorsalis pedis arteries; and plasma levels were measured before and after the 7 weeks treatment.

Enhanced external counterpulsation (EECP) is widely utilized in rehabilitating patients after percutaneous coronary intervention (PCI) and has demonstrated efficacy in promoting cardiovascular function recovery. Although the precise mechanisms of the therapeutic effects remain elusive, it is widely postulated that the improvement of biomechanical environment induced by EECP plays a critical role. This study aimed to unravel the underlying mechanism through a numerical investigation of the in-stent biomechanical environment during EECP using an advanced multi-dimensional 0/1D-3D coupled model.

BACKGROUND Congenital pericardial agenesis is a rare condition that is frequently associated with abnormal intrathoracic anatomy, especially malpositioning of the heart within the thoracic cavity. In the setting of coronary artery bypass, these anatomic derangements can present surgical challenges that can necessitate incorporating complementary, non-surgical solutions for complete revascularization. CASE REPORT A 48-year-old male patient presented with acute anginal symptoms, with workup revealing severe, multivessel coronary artery disease, as well as partial absence of the pericardium.

Background: Our study aims to compare in-hospital management and outcomes in patients with cardiogenic shock due to ST-segment elevation myocardial infarction (STEMI) versus non-ST-segment elevation myocardial infarction (NSTEMI). Methods: We conducted a retrospective cohort study using the National Inpatient Sample database between 2016-2019, including patients with STEMI/NSTEMI complicated by cardiogenic shock. An inverse probability treatment weighting analysis was performed to compare in-hospital management and outcomes between patients with STEMI and NSTEMI.

Background: The role of intra-aortic balloon pump (IABP) combined with venoarterial extracorporeal membrane oxygenation (VA-ECMO) in patients with cardiogenic shock (CS) remains unclear. This study investigated the effect of applying IABP for left ventricle (LV) unloading after VA-ECMO on reducing mortality in patients with CS.
Methods: Data from 5,492 consecutive patients with CS treated with VA-ECMO between January 2017 and July 2023 were collected from the CSECLS registry.
